摘要:
目的：探讨5-氨基酮戊酸光动力对手术治疗基底细胞癌的疗效及对免疫系统的影响。方法：选取2018年6月-2020年6月笔者医院收治的基底细胞癌患者164例，随机分为对照组和观察组各82例。对照组接受单纯手术治疗，观察组在对照组基础上接受5-氨基酮戊酸光动力治疗。评价治疗干预后的美容效果、疗效和免疫指标变化等。结果：治疗后，观察组治疗总优良率（93.90%）显著高于对照组（75.61%），总有效率（90.24%）显著高于对照组（69.51%），差异均有统计学意义（P＜0.05）。观察组CD4+、CD4+/CD8+显著高于对照组，观察组患者血清IL-1（8.34±2.41）、TNF-α（51.62±10.17）的水平低于对照组。观察组总不良反应发生率（13.41%）显著低于对照组（28.05%），差异有统计学意义（P＜0.05）。结论：5-氨基酮戊酸光动力联合手术治疗基底细胞癌疗效较好，不良反应少，美容效果好，可能与有效改善患者免疫功能有关。
Abstract:
Objective To investigate the effect of photodynamics of 5-aminolevulinic acid on the efficacy of surgical treatment of basal cell carcinoma and its influence on the immune system. Methods A total of 164 basal cell carcinoma patients admitted to our hospital from June 2018 to June 2020 were selected and randomly divided into control group and observation group with 82 cases each. The control group received surgical treatment alone, and the observation group received 5-aminolevulinic acid photodynamic therapy on the basis of the control group. Evaluate the cosmetic effect, curative effect, immune index changes and the occurrence of adverse reactions after treatment intervention. Results After treatment, the total excellent and good rate of clinical treatment in the observation group (93.90%) was higher than that in the control group (75.61%), and the total effective rate of clinical treatment in the observation group (90.24%) was higher than that of the control group (69.51%). The difference was statistically significant. Significance (P ＜0.05). The observation group’s CD4+, CD4+/CD8+ were significantly higher than the control group; the observation group’s serum IL-1 (8.34±2.41) and TNF- α (51.62±10.17) levels were lower than the control group. The incidence of total adverse reactions in the observation group (13.41%) was lower than that in the control group (28.05%), and the difference was statistically significant (P ＜0.05). Conclusion The combination of 5-aminolevulinic acid and photodynamic surgery in the treatment of basal cell carcinoma has a good effect, few adverse reactions and good cosmetic effects, which may be related to the effective improvement of the patient’s immune function.
