摘要:
目的：观察改良式上睑提肌缩短术对上睑下垂疗效，及对患者眼睑美观度、上睑缘遮盖角膜距离的影响。方法：选取2018年9月-2020年5月笔者医院收治的先天性上睑下垂患者97例作为研究对象，均给予改良式上睑提肌缩短术。记录患者治疗过程中不同时间段的上睑缘遮盖角膜距离及眼睑闭合程度，观察所有患者并发症发生情况。结果：术前、术后3个月、术后6个月及术后12个月患者平均上睑缘遮盖角膜的距离分别为（3.95±0.82）mm、（1.65±0.14）mm、（1.68±0.16）mm、（1.70±0.15）mm，术前与术后比较，差异有统计学意义（P＜0.05）。术前、术后3个月、术后6个月及术后12个月患者上睑肌肌力分别为（3.86±1.09）mm、（8.09±1.02）mm、（10.78±1.87）mm、（11.84±2.57）mm，术前与术后比较差异有统计学意义（P＜0.05）。术后满意率为94.44%，良好率为4.17%，差率为1.39%。对所有患者进行随访1年，均未发现倒睫、暴露性角膜炎、结膜脱垂、感染、上睑迟滞及眼睑外翻等并发症。结论：改良式上睑提肌缩短术治疗上睑下垂具有良好的疗效且并发症发生率低，且患者满意度高。
Abstract:
Objective The effect of modified eyelid muscle shortening on ptosis and the effect of eyelid aesthetics and corneal distance were observed. Methods Ninety patients with congenital ptosis, 59 males and 38 females were admitted to the hospital from September 2018 to May 2020.Revised upper eyelid muscle shortening were performed.The eyelid aesthetics, upper corneal distance were observed and recorded before, 3 months, 6 months and 12 months after surgery.Complications such as exposed keratitis, inverted eyelashes, conjunctival prolapse, upper eyelid retardation, and infection were recorded in all patients.Results The distance between the mean upper eyelid covered cornea, before, 3 months, 6 months and 12 months, was (3.95±0.82) mm, (1.65±0.14)mm,(1.68±0.16)mm, (1.70±0.15)mm.The upper bleral muscle strength of patients before surgery, 3 months after surgery, 6 months after surgery and 12 months after surgery was (3.86±1.09)mm, (8.09±1.02)mm, (10.78±1.87)mm, (11.84±2.57)mm, respectively, and the difference between preoperative and postoperative was statistically significant (P＜0.05). Postoperative satisfaction was 94.44%, good rate was 4.17% and difference rate was 1.39% full.All patients follow-up for 1 year revealed no complications such as inverted eyelashes, exposed keratitis, conjunctival prolapse, infection, upper eyelid retardation, and eyelid valgus. Conclusion Revised ptoeyelid shortening for ptosis has good efficacy, low incidence of complications and high patient satisfaction
