摘要:
目的：观察基于躯体-呼吸放松训练的心理干预联合舒适护理在手部深度烧伤皮瓣移植患者中的应用效果。方法：选 取2018年1月-2021年5月笔者医院收治的66例手部深度烧伤皮瓣移植患者为研究对象，依照区组随机化分组法将其分为对照 组和观察组，各33例。对照组使用常规护理干预，观察组则在其基础上实施基于躯体-呼吸放松训练的心理干预联合舒适护 理干预。比较两组术后基本情况（皮瓣移植面积、创面愈合时间、住院时间）及手部美观情况[瘢痕美容评估与评级（Scar cosmesis assessment and rating，SCAR）]，对比干预前、干预3个月后两组手部功能情况（手功能评定表）和生活质量[烧 伤患者健康量表（Burn specific health scale，BSHS）]。结果：两组皮瓣移植面积比较差异无统计学意义（P＞0.05）。观 察组创面愈合时间、住院时间均明显低于对照组，差异有统计学意义（P＜0.05）。干预3个月后，两组手功能评定表中手部 活动、日常活动、感觉功能与外观三个重要维度评分及其总分较干预前均有明显升高，且观察组明显高于同一时间对照组， 差异有统计学意义（P＜0.05）。观察组SCAR各项目评分及其总分均明显低于对照组，差异有统计学意义（P＜0.05）。两组 BSHS中日常活动、心理状态维度评分及其总分较干预前均有明显升高，且观察组明显高于同一时间对照组，差异有统计学 意义（P＜0.05）。结论：手部深度烧伤皮瓣移植患者应用基于躯体-呼吸放松训练的心理干预联合舒适护理能加快其术后恢 复，保持患者手部美观，提高患者生活质量，具有较高临床应用价值。
Abstract:
Objective To observe the application effect of psychological intervention based on body-breathing relaxation training combined with comfort nursing on patients with hand deep burn undergoing skin fl ap transplantation. Methods 66 patients with hand deep burn undergoing skin flap transplantation admitted to the hospital between January 2018 and May 2021 were selected as the research subjects, and they were divided into the control group and the observation group according to the block randomized grouping method, with 33 cases in each group. The control group received conventional nursing intervention, and the observation group implemented psychological intervention based on body-breathing relaxation training combined with comfort nursing intervention on this basis. The postoperative basic conditions (skin flap transplantation area, wound healing time, hospitalization time) and hand beauty status (SCAR) of the two groups were compared. The hand function status (hand function assessment scale) and quality of life (BSHS) were compared between the two groups before intervention and 3 months after intervention. Results There was no significant difference in the area of flap transplantation between the two groups (P＞0.05). The wound healing time and hospitalization time of the observation group were significantly lower than those of the control group, and the differences were statistically significant (P＜0.05). After 3 months of intervention, the scores of three important dimensions of hand activity, daily activities and sensory function and appearance and total score of hand function assessment scale in the two groups were significantly increased compared with those before intervention (P＜0.05). And the scores in the observation group were significantly higher than those in the control group at the same time point (P＜0.05). The scores of various items and total score of SCAR scale in the observation group were significantly lower than those in the control group (P＜0.05). The scores of dimensions of daily activities and psychological status and total score of BSHS scale in the two groups were significantly increased compared with those before intervention (P＜0.05). And the scores of the observation group were significantly higher than those in the control group at the same time point (P＜0.05). Conclusion The application of psychological intervention based on body-breathing relaxation training combined with comfort nursing for patients with hand deep burn undergoing skin flap transplantation can speed up the postoperative recovery, keep the hand beauty and improve the quality of life of patients, with high clinical application value.
