摘要:
目的：比较金属烤瓷冠与氧化锆全瓷冠在牙体缺损修复中的美学效果及对牙周健康的影响。方法：选取2019年 1 月-2022年1月笔者医院接受种植牙修复的80例患者（共90颗患牙）为研究对象，随机分为观察组和对照组。观察组 （n =40，患牙46颗）采用氧化锆全瓷冠修复，对照组（n =40，患牙44颗）采用钴铬合金烤瓷全冠修复，比较两组临床 恢复时间、修复前与修复6个月后种植体牙周指标，以及周围龈沟液（Gingival crevicular fluid，GCF）中超氧 化物歧化酶（Superoxide dismutase，SOD）、干扰素-γ（Interferon-γ，IFN-γ）、基质金属蛋白酶2（Matrix metalloproteinase-2，MMP-2）、谷胱甘肽过氧化物酶（Glutathione peroxidase，GP-x）的表达水平，对比随访12个 月的远期效果。结果：观察组恢复时间短于对照组（P＜0.001）；修复6个月后，观察组菌斑指数、龈沟出血指数均低 于对照组，而牙周探诊深度高于对照组（P＜0.05）；修复后，两组GCF各指标发生明显变化，且观察组修复6个月后GCF中 SOD 、GP-x水平高于对照组，IFN-γ、MMP-2水平低于对照组（P＜0.05）；随访12个月，观察组牙周炎、牙龈炎发生率低于 对照组（P＜0.05），观察组修复体外形、颜色匹配、牙龈适应性及牙敏感性合格率均高于对照组（P＜0.05）。结论：与钴 铬合金烤瓷全冠修复相比，氧化锆全瓷冠修复可明显改善种植体牙周指标及GCF中SOD、IFN-γ、MMP-2、GP-x表达水平，且 远期效果也较好，值得在临床推广应用。
Abstract:
Objective The purpose of this study was to compare the aesthetic eff ects and periodontal health of porcelain crown and all-ceramic crown restorations. Methods A total of 80 patients (90 teeth) who underwent dental implant restoration in the hospital from January 2019 and January 2022 were randomly divided into the observation group (n=40, 46 teeth, repaired with zirconia all-ceramic crown) and control group (n=40, 44 teeth, repaired with Co-Cr PFM). Clinical treatments, periodontal indicators, the levels of superoxide dismutase (SOD), interferon-γ (IFN-γ), matrix metalloproteinase 2 (MMP-2) and glutathione peroxidase (GP-x) in gingival crevicular fl uid (GCF) were compared between the two groups. The long-term eff ects were compared. Results The recovery time in the observation group were shorter than those in the control group (P ＜0.001). 6 months after repair, plaque index (PLI) and sulcus bleeding index (SBI) in the observation group were lower than those in the control group, while probing depth (PD) was higher than that in the control group (P＜0.05). After restoration, all GCF indicators changed signifi cantly. 6 months after restoration, the levels of SOD and GP-x in GCF of the observation group were higher than those in the control group, while the levels of IFN -γ and MMP-2 were lower than those in the control group (P＜0.05). During the 12-month follow-up, the incidence rates of periodontitis and gingivitis in the observation group were lower than those in the control group (P＜0.05). The qualifi cation rates of restoration appearance, color matching, gingival adaptability and tooth sensitivity in the observation group were higher than those in the control group (P ＜0.05). Conclusion Compared with Co-Cr PFM restoration, zirconia all-ceramic crown restoration can signifi cantly improve periodontal indicators and the expression levels of SOD, IFN-γ, MMP-2 and GP-x in GCF, with good long-term eff ect.
