[摘要]目的：观察右美托咪定和舒芬太尼联合肿胀麻醉用于美容整形手术的临床效果。方法：100例患者分为A、B两组。A组静
注右美托咪定1μg/kg(10min注入），维持量为0.3～0.5μg/(kg·h)，B组静注丙泊酚1.5～2mg/kg，维持量为3～5mg/(kg·h)。
手术开始前5min两组患者皆静注0.2μg/kg舒芬太尼，实施肿胀麻醉。观察并记录麻醉前（T0）、麻醉后（注射舒芬太尼
后）（T1）、肿胀麻醉开始时（T2）、手术30min时（T3）、手术结束时（T4）、清醒时（T5）的HR、RR、MAP、SpO2，患者
诉疼痛难忍（或体动影响手术）、呼吸抑制情况，清醒时间（术毕至steward评分达6分时间)，术后30min视觉模拟评分法
(Visual analogue scale，VAS)评分情况及手术时间。结果：与T０时比较，T1时两组HR、RR、MAP均明显降低，T2、T3时
B组HR快于、RR慢于、MAP高于A组，差异均具有统计学意义（P ＜0.05）。A组术毕清醒时间、术中呼吸抑制、出现疼痛
或体动的发生率明显低于B组，差异有统计学意义(P ＜0.05）；两组间舒芬太尼用量、VAS评分比较，差异无统计学意义
（P ＞0.05）。结论：右美托咪定联合肿胀麻醉用于美容整形手术，优于丙泊酚麻醉，是一种理想的麻醉方法。
Abstract:
Abstract: Objective To observe the clinical effect of dexmetomidine and sufentanil combined with swelling anesthesia in
cosmetic surgery. Methods One hundred patients were divided into two groups: group A received intravenous injection of
1μg/kg (10 min), the maintenance dose was 0.3-0.5μg/(kg?h),group B intravenous propofol 1.5-2mg/kg, and the maintenance
dose was 3-5mg /(kg?h). 0.2μg/kg sufentanil was given intravenously 5 minutes before the operation and swelling anesthesia
was performed in both groups. Observed and recorded pre-anesthesia (T0), post-anesthesia (after sufentanil) (T1), swelling
anaesthesia began (T2), 30 minutes after operation (T3), at the end of operation (T4), and awake (T5) complained of pain (or
body movement affecting surgery), respiratory inhibition, Awake time (from the end of operation to steward score six), 30
minutes after operation VAS score, operation time. Results Compared with T0, HR, RR and MAP of the two groups decreased
significantly at T1, HR of group B was faster, RR was slower and MAP was higher at T2 and T3 than that of group A, the
differences were statistically significant(P ＜0.05). The incidence of awakening time, intraoperative respiratory depression, pain
or physical movement in group A was significantly lower than that in group B(P ＜0.05). There was no significant difference
in sufentanil dosage and VAS score between the two groups(P ＞0.05). Conclusion Dexmetomidine combined with swelling
anesthesia is superior to propofol anesthesia in cosmetic surgery and is an ideal anaesthesia method.
