[摘要]目的:探讨自体贫血小板血浆(Platelet-poor plasma,PPP)结合富血小板纤维蛋白(Platelet-rich fibrin,
PRF)在恒牙撕脱伤治疗中的应用。方法:选取2017年10月-2019年6月笔者医院收治的恒牙撕脱性损伤并行牙再植术的患者
150例,分为A、B、C组,每组50例。A组采用常规再植,B组于再植前向牙槽窝植入PRF颗粒,C组于再植前采用PPP浸泡脱位
牙并向牙槽窝植入PRF颗粒。三组均弹性固定4周,随访12个月。12个月后进行疗效评价,术后次日、术后4周和随访结束
时,患者均接受生活质量综合评定问卷(GQOLI-74)评分,并记录并发症。结果:A组再植牙的总有效率为76.36%,B组再植
牙的总有效率为87.93%,C组再植牙的总有效率为93.10%(P <0.05)。术后4周,三组GQOLI-74的躯体功能、心理功能、社
会功能及物质生活四个维度评分均高于同组术后次日,且B组高于A组,C组高于A组和B组(P <0.05)。随访结束时,三组
GQOLI-74的四个维度评分均高于同组术后次日和术后4周,且B组高于A组,C组高于A组和B组(P <0.05)。C组并发症发生率
均低于A组和B组(P <0.05)。结论:自体PPP和PRF可以有效用于恒牙撕脱伤后再植,联合应用疗效更好。

Abstract:
Abstract: Objective To investigate the clinical application of autologous platelet-poor plasma (PPP) combined with plateletrich
fibrin (PRF) in the treatment on avulsion of permanent teeth. Methods Total of 150 patients with avulsion injury of
permanent teeth and replantation admitted to our hospital from October 2017 to June 2019 were selected and divided into group
A (n =50), group B (n =50) and group C (n =50). Group A was treated with conventional replantation, group B was treated with
PRF particles into the alveolar fossa before replantation, and group C was treated with PPP to soak the displaced teeth and
PRF particles into the alveolar fossa before replantation. All three groups were fixed flexibly for 4 weeks and followed up for
12 months. Efficacy was evaluated 12 months later. The day after surgery, 4 weeks after surgery and at the end of follow-up,
patients were received generic quality of life inventory 74 (GQOLI-74) score, and complications were recorded. Results The
total effective rate was 76.36% in group A, 87.93% in group B, and 93.10% in group C (P <0.05). Four weeks after surgery, the
GQOLI-74 scores in the three groups were all higher than those in the same group the day after surgery, and group B was higher
than group A, and group C was higher than group A and group B (P <0.05). At the end of follow-up, the GQOLI-74 scores
in the three groups were higher than those in the same group the day after and 4 weeks after surgery, and group B was higher
than that in group A, and group C was higher than that in group A and group B (P <0.05). The incidence of complications in
group C was lower than group A and group B (P <0.05). Conclusion The autologous PPP and PRF can be effectively used for
replantation after avulsion of permanent teeth, and the combined application has better efficacy.
