摘要:

[摘要]目的：探究改良远离角膜缘结膜瓣移植治疗翼状胬肉的美容效果。方法：选择2018年2月-2020年9月笔者医院收治的
翼状胬肉患者66例（85只眼）作为研究对象，采用随机数字表法分为两组，对照组33例（44只眼）采用常规角膜缘结膜瓣移
植治疗，观察组33例（41只眼）采用改良远离角膜缘结膜瓣移植，自术眼颞上方距离角膜缘3mm取结膜瓣移植。比较两组患
者术前、术后1个月、3个月泪液分泌试验（SIT）、泪膜破裂时间（BUT）、角膜荧光素染色（CSF）情况，评估术后30d美容
效果及复发情况，统计两组满意度。结果：术后1个月、3个月观察组SIT、BUT均高于对照组，CSF低于对照组，差异有统计
学意义（P ＜0.05）。两组患者SIT、BUT在时间及组别上对比具有明显差异，而且时间与组间具有交互作用（P ＜0.05）。观
察组美容优良率较对照组高（90.24% vs 70.45%），差异有统计学意义（P＜0.05）。观察组复发率与对照组比较差异无统
计学意义（7.32 % vs 9.09%，P＞0.05）。观察组操作技能、服务态度、视力改善、外观改善满意度评分较对照组高，差异
有统计学意义（P ＜0.05）。结论：改良远离角膜缘结膜瓣移植用于翼状胬肉治疗，对眼表结构及功能影响较常规角膜缘结
膜瓣移植法小，复发率低，外观恢复快，可提高美容效果及患者满意度，值得推广。

Abstract:

Abstract: Objective To investigate the effectiveness of modified conjunctiva flap transplantation far away from corneal limbus
in the treatment of pterygium. Methods A total of 66 pterygium patients (85 eyes) admitted to our hospital from February 2018
to September 2020 were selected as study subjects and divided into two groups by random number table method. 33 patients
(44 eyes) in the control group were treated with conventional limbal conjunctival flap transplantation, 33 patients (41eyes) in
the observation group were treated with modified conjunctival flap transplantation away from the limbal of the cornea, and
conjunctival tissue was taken from the right eye above the temporal distance of 3mm from the limbal of the cornea. The schirmer I
test (SIT), break-up time (BUT), and corneal fluorescein staining (CSF) were compared between the two groups before operation,
1 month and 3 months after operation. The cosmetic effect and recurrence 30 days after operation were evaluated, and the
satisfaction of the two groups was counted. Results At 1 and 3 months after operation, SIT and BUT in the observation
group were higher than those in the control group, and CSF was lower than that in the control group, the differences were
statistically significant (P ＜0.05). There were significant differences in time and group between the two groups, and there was
interaction between time and group (P ＜0.05). The excellent and good rate of beauty in the observation group was higher than
that in the control group (90.24% vs 70.45%, P ＜0.05). There was no significant difference in the recurrence rate between the
observation group and the control group (7.32% vs 9.09%, P ＞0.05). The satisfaction scores of operation skills, service attitude,
vision improvement and appearance improvement in the observation group were higher than those in the control group (P ＜0.05).
Conclusion Modified conjunctival flap transplantation far from corneal limbus in the treatment of pterygium has less effect
on ocular surface structure and function than conventional conjunctival flap transplantation, with lower recurrence rate, faster
appearance recovery, better cosmetic effect, and thus better satisfaction. It is worth promoting.
