摘要:

目的：探讨胬肉切除联合自体结膜移植术治疗翼状胬肉的疗效，以及对泪膜功能、屈光变化的影响。方法：选择 2019 年1月-2020年12月因翼状胬肉入院的60例患者作为研究对象，并随机分为A组和B组，每组30例。A组采用切除翼状胬 肉联合结膜移植术，B组则为单纯切除翼状胬肉，观察比较术后并发症发生情况、术前与术后6个月的美学效果、泪膜功能 及屈光状态改变情况。结果：治疗后A组的结膜肉芽肿和干眼症的发生例数均少于B组，两组数据比较差异具有统计学意义 （P＜0.05）；A组的治愈例数高于B组，复发例数明显少于B组，差异具有统计学意义（P＜0.05）；两组泪膜破裂时间 （BUT）、基础泪液分泌实验（SIT）、眼表疾病指数(OSDI)和角结膜荧光素染色(CFS)实验术前比较差异无统计学意义 （P＞0.05），术后A组数据均优于B组，差异有统计学意义（P＜0.05），且两组术后与同组术前比较，均有较好的效果，差 异具有统计学意义（P＜0.05）；术前两组裸眼视力（UCVA）、角膜散光度(CAD)、角膜垂直曲度和角膜水平曲度检测结果比 较差异无统计学意义（P＞0.05），术后A组数据均优于B组，差异具有统计学意义（P＜0.05），且两组术后与同组术前的数 据比较，差异具有统计学意义（P＜0.05）。结论：手术切除联合结膜移植术治疗翼状胬肉术后效果明显优于单纯胬肉切除 术，且前者在预防复发、泪膜功能和屈光状态方面较后者具有较大优势。

Abstract:

Objective To explore the cosmetic eff ect of pterygium excision combined with conjunctival transplantation in the treatment of pterygium and its infl uence on tear fi lm function and refraction change. Methods 60 cases admitted to the hospital due to pterygium from January 2019 to December 2020 were selected as the research subjects, and randomly divided into group A and group B, with 30 cases in each group. Group A was combined with conjunctival transplantation to surgically remove the pterygium, and the group B was simply removed pterygium. The postoperative complications and changes in aesthetic eff ect, tear fi lm function and refraction status before surgery and at 6 months after surgery were observed and recorded. Results The number of cases of conjunctival granuloma and xerophthalmia in group A were less than those in group B (P ＜0.05). The number of cured cases in group A was more than those in group B while the number of cases with recurrence was signifi cantly less than that in group B (P ＜0.05). The tear fi lm break-up time (BUT), Scirmer I test (SIT), ocular surface disease index (OSDI) and corneal conjunctiva fl uorescein staining (CFS) showed no statistically signifi cant diff erences between the two groups before surgery (P ＞0.05), and the above data in group A were better than those in group B after surgery (P＜0.05), and the comparison of postoperative and preoperative data in groups A and B showed good results (P＜0.05). The uncorrected visual acuity (UCVA), corneal astigmatism (CAD), corneal vertical curvature and corneal horizontal curvature showed that there were no statistical diff erences between the two groups before surgery (P ＞0.05), and the postoperative data in group A were better than those in group B (P＜0.05), and there were statistical diff erences in the data of group A and group B after surgery compared with those before surgery (P ＜0.05). Conclusion Pterygium excision combined with conjunctival transplantation is signifi cantly better than pterygium excision in the aesthetic eff ect of pterygium treatment. The former one has greater advantages than the latter one in terms of recurrence prevention, tear fi lm function and refraction status.
