[摘要]目的:观察光子嫩肤联合氨甲环酸治疗黄褐斑的临床疗效,并分析影响疗效的相关因素。方法:选取2018年4月-2019 年4月笔者医院收治的72例黄褐斑患者为研究对象,随机分为观察组(36例)和对照组(36例)。对照组采用氨甲环酸治 疗,观察组在对照组的基础上加用光子嫩肤治疗,疗程均为3个月。治疗后,评价两组疗效。治疗前、治疗1个月、3个月 后,两组行黄褐斑面积及严重程度(MASI)评分,并记录不良反应。采用单因素和多因素Logistic回归模型分析影响患者疗 效的相关因素。结果:观察组总有效率86.11%,高于对照组的63.89%(P<0.05)。治疗1个月后,两组MASI评分均低于治疗 前,且观察组低于对照组;治疗3个月后,两组MASI评分均低于治疗前和治疗1个月后,且观察组低于对照组(P<0.05)。 两组不良反应发生率组间比较,差异无统计学意义(P>0.05)。Wood灯下分型、临床分期、防晒霜使用和治疗方式是疗效 的独立影响因素(P<0.05)。结论:光子嫩肤联合氨甲环酸能够有效治疗黄褐斑,黄褐斑Wood灯下为表皮型、处于稳定 期、保持良好防晒和光子嫩肤联合氨甲环酸治疗是促进黄褐斑疗效的有利因素。
Abstract:
Abstract: Objective To investigate the effi cacy of photorejuvenation technology combined with tranexamic acid on patients with chloasma, and to analyze the infl uencing factors of effi cacy. Methods Total of 72 patients with chloasma admitted to our hospital from April 2018 to April 2019 were randomly divided into the observation group (n=36) and the control group (n=36). The control group was treated with tranexamic acid. On the basis of the control group, the observation group was treated with photorejuvenation technology. The course of treatment in two groups was 3 months. After treatment, the effi cacy in two groups was evaluated. Before treatment, after 1 month of treatment and after 3 months of treatment, the melasma area severity index (MASI) was estimated in two groups, and adverse reactions were recorded. Univariate analysis and multiple Logistic regression models were used to analyze the infl uencing factors of effi cacy. Results The total eff ective rate of the observation group was 86.11%, which was higher than 63.89% of the control group (P<0.05). After 1 month of treatment, MASI score in two groups were lower than those before treatment, and the MASI score in the observation group was lower than that in the control group. After 3 months of treatment, MASI score in two groups was lower than those before treatment and after 1 month of treatment, and the MASI score in the observation group was lower than those in the control group (P<0.05). There was no signifi cant difference in the incidence of adverse reactions between the two groups (P>0.05). The pathological type of Wood lamp, clinical stage, use of sun cream and treatment were independent infl uencing factors of effi cacy (P<0.05). Conclusion The photorejuvenation technology combined with tranexamic acid can eff ectively cure chloasma. Chloasma is epidermal type, in stable period, keeping sunscreen and treatment of photorejuvenation technology combined with tranexamic acid are favorable factors to promote the effi cacy.?/html>
