摘要:
目的:探讨超脉冲点阵CO2激光联合米诺地尔治疗斑秃的疗效。方法:选取2018年1月-2020年11月于笔者医院就诊的124例斑秃患者,采用随机数字表法分为对照组和联合组,每组62例。对照组仅予以5%米诺地尔外用治疗,联合组行10600nm超脉冲点阵CO2激光联合外用5%米诺地尔治疗,两组均治疗6周。对比分析两组治疗前后毛发镜检查结果和干扰素-γ(IFN-γ)、肿瘤坏死因子α(TNF-α)、白细胞介素12(IL-12)及白细胞介素18(IL-18)等炎症因子水平,对两组的治疗及美容效果进行评价,并统计治疗期间不良反应发生情况。结果:治疗后,联合组总有效率91.94%,对照组为79.03%,联合组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组的毛囊密度、毛发密度均较治疗前提高,毳毛率较治疗前下降,差异有统计学意义(P<0.05),且联合组的变化幅度大于对照组(P<0.05)。治疗后,联合组的IFN-γ、TNF-α、IL-12以及IL-18水平均低于对照组,差异有统计学意义(P<0.05)。治疗期间,两组不良反应发生率分别为8.07%和11.30%,差异无统计学意义(P>0.05)。治疗后,联合组的美容效果评分高于对照组,差异有统计学意义(P<0.05)。结论:10600nm超脉冲点阵CO2激光联合米诺地尔治疗斑秃可促进斑秃部位毛囊以及毛发的生长,抑制机体的炎症反应,不良反应轻,效果显著。
Abstract:
Objective To investigate the effect of ultra-pulsed carbon dioxide fractional laser combined with minoxidil in the treatment of alopecia areata. Methods A total of 124 patients with alopecia areata treated in the hospital from January 2018 to November 2020 were selected and divided into control group and combined group by random number table method, with 62 cases in each group. The control group only received external application of 5% minoxidil, while the combined group received treatment with 10?600nm ultra-pulsed carbon dioxide fractional laser and external application of 5% minoxidil. Both groups were treated for 6 weeks. The trichoscopy results, levels of interferon- γ (IFN- γ), tumor necrosis factor α (TNF- α), interleukin 12 (IL-12) and interleukin 18 (IL-18) before and after treatment were compared between the two groups. The therapeutic and cosmetic effects of the two groups were evaluated, and the incidence of adverse reactions during treatment was counted. Results The total response rate in the combined group was 91.94%, which was higher than 79.03% of the control group (P<0.05). After treatment, the hair follicle density and hair density were improved, and the vellus hair rate was reduced in the two groups (P <0.05). The above changes were greater in the combined group than in the control group (P<0.05). After treatment, the levels of IFN-γ, TNF-α, IL-12 and IL-18 in the combined group were lower than those in the control group (P<0.05). The incidences of adverse reactions in the two groups during treatment were 8.07% and 11.30%, respectively (P>0.05). After treatment, the mean cosmetic score of the combined group was higher than that of the control group (P<0.05). Conclusion 10?600nm ultra-pulsed carbon dioxide fractional laser combined with minoxidil can promote the growth of hair follicles and hairs in the alopecia areata area, and inhibit inflammatory response, with mild adverse reactions and significant effects.
