摘要:
目的:探讨Twin-block矫治器治疗早期下颌后缩畸形的美学效果及对牙齿咬合力的影响。方法:选择2018年1月-2019年6月笔者医院诊治的42例安氏Ⅱ1类错牙合畸形下颌后缩患者为研究对象,以随机数字表法分为两组,对照组(采用传统直丝弓系统矫治)21例,观察组(采用Twin-block矫治器)21例。记录两组患者治疗时间,比较两组患者治疗前后牙齿咬合力、咀嚼功能的变化,记录两组患者治疗前后颌面部形态指标及软硬组织指标变化情况。结果:对照组治疗(15.15±2.13)个月,观察组治疗(10.12±1.06)个月,观察组治疗时间显著低于对照组,差异有统计学意义(P<0.05);治疗后,观察组牙齿咬合力、咀嚼功能高于对照组,差异有统计学意义(P<0.05);治疗后,观察组下颌骨长、下面高度、上下齿槽角及上下齿槽座夹角的改善均好于对照组,差异均有统计学意义(P<0.05);治疗后,观察组上切牙露齿度、软组织面角、鼻唇角、面突角及上、下颌平面角显著高于对照组,下颌头影指标、覆牙合、覆盖及上、下唇审美平面距改善好于对照组,差异有统计学意义(P<0.05)。结论:Twin block矫治器治疗安氏Ⅱ1类错牙合畸形可促进下颌骨向前生长,抑制上颌骨的生长。协调矢状向上下颌骨的位置关系,提高牙齿咬合力,减小前牙覆盖、覆牙合,改善颜面软组织的美观与比例。
Abstract:
Objective To explore the aesthetic effect of twin block appliance in the treatment of early mandibular retrusion and the influence of tooth bite force. Methods 42 cases of Class Ⅱ Division 1 malocclusion patients with mandibular retrusion in our hospital from January 2018 to June 2019 were selected and randomly divided into control group (traditional straight wire system) and observation group (twin block appliance) with 21 cases. The treatment time of the two groups was recorded. The changes of bite force and chewing function of the two groups were compared before and after treatment The changes of maxillofacial morphology indexes and soft and hard tissue indexes of the two groups before and after treatment were recorded. Results The treatment time of the control group was(15.15±2.13) months, and the treatment time of the observation group was (10.12±1.06) months.The treatment time of the observation group was significantly lower than that of the control group (P<0.05). After treatment, the bite force and masticatory function of the observation group were higher than those of the control group (P<0.05). After treatment,the improvement of mandible length, lower height, upper and lower alveolar angle and angle between upper and lower alveolar seats in the observation group were better than those in the control group, and the differences were statistically significant (P<0.05). After treatment, the degree of upper incisor, soft tissue facial angle, nasolabial angle, facial protrusion angle, and upper and mandibular plane angles of the observation group were significantly higher than those of the control group. The index of mandibular head shadow, overbite, covering, and upper and lower lip aesthetics The plane distance improved better than the control group, the difference was statistically significant (P<0.05). Conclusion The Twin-block appliance can promote the forward growth of the mandible and inhibit the growth of the maxillary in the treatment of Class Ⅱ Division 1 malocclusion. Coordinate the positional relationship of the sagittal upper mandible, improve the occlusal force of the teeth, reduce the coverage and overbite of the anterior teeth, and improve the beauty and proportion of the facial soft tissues.
