摘要:

目的：研究不同皮瓣修复重建颌面部肿瘤切除术后的疗效及对患者口腔闭合功能和外形恢复情况的影响。方法：选择2012年3月-2019年3月在笔者医院治疗的120例颌面部肿瘤患者为研究对象，按照随机分组原则，将患者随机分为A、B、C三组，每组40例。A组患者前臂皮瓣进行修复，B组患者采取股前外侧皮瓣进行修复，C组患者采取串联皮瓣修复。对三组患者的手术指标、术后恢复情况以及并发症进行比较。结果：三组患者的手术时间、皮瓣制作以及吻合时间、拔出胃管时间、经口进食时间比较，差异存在统计学意义（P＜0.05），经过两两比较，手术时间、拔出胃管时间、经口进食时间从高到低依次为C组、B组、A组，皮瓣制作以及吻合时间从高到低分别为C组、A组、B组；三组患者的满意度，外形修复、工作情况、口腔闭合、咀嚼功能、吞咽功能、咬合功能及语言功能评分从高到低依次为A组、B组、C组；三组患者的瘢痕增生、瘙痒、色素沉着、植皮区域麻木、植皮区域异常等情况比较，差异有统计学意义（P＜0.05）。结论：经前臂皮瓣修复的颌面部肿瘤术后创面,术后患者口腔闭合功能、外形恢复情况较好，患者的疗效较好。

Abstract:

Objective To study the effect of different flap reconstruction on the postoperative curative effect of patients with maxillofacial tumor and the recovery of oral closure function and appearance. Methods 120 cases of maxillofacial tumor patients who were treated in our hospital from March 2012 to March 2019 were taken as the research objects. According to the inpatient number and the principle of random grouping, the above patients were randomly divided into three groups: group A, group B and group C with 40 patients in each group. Group A with forearm flap, group B with anterolateral thigh flap and group C with serial skin Flap repair. The operative indexes, postoperative recovery and complications of the three groups were compared. Results There were statistically significant differences in operation time, flap making, anastomotic time, gastric tube pulling out time and oral feeding time among the three groups (P＜0.05). After the comparison of the two groups, the operation time, the time of pulling out the gastric tube and the time of eating by mouth were in group C, group B and group A, respectively.The flap making and anastomosis time from high to low were group C,group A and group B respectively.The satisfaction, shape repair, working condition, mouth closure, chewing function, swallowing function, occlusal function and language function of the three groups were in group A from high to low Group B, group C,There were significant differences among the three groups in scar hyperplasia, pruritus, pigmentation, numbness and abnormality of skin grafting area (P＜0.05). Conclusion Patients with maxillofacial tumors repaired by forearm flap have better oral closure function and shape recovery, and the curative effect of patients is better.
