摘要:
目的：探讨加味补肺汤联合针灸疗法治疗肺气亏虚型系统性硬皮病的疗效及对血清免疫因子的影响。方法：选择2017年3月-2020年3月黔江中心医院皮肤科收治的系统性硬皮病患者为研究对象，共80例，通过随机数字表法分为试验组及对照组，每组40例。对照组给予青霉胺片、维生素B6片及维生素B1片进行治疗；试验组在对照组的基础上给予加味补肺汤联合针灸疗法进行治疗，两组均持续治疗24周。比较两组治疗24周后的疗效，比较治疗前、治疗24周后的中医证候、皮肤硬度、关节痛、关节功能积分及免疫功能。结果：治疗24周后，试验组总有效率为95.00%，高于对照组的72.50%，差异有统计学意义（P＜0.05）。与治疗前比较，治疗24周后两组舌苔薄白、神疲乏力、畏寒肢冷、腰酸无力积分、中医证候总积分、皮肤硬度、关节痛、关节功能积分及外周血CD8+水平均降低，试验组低于对照组；而外周血CD4+、CD4+/CD8+水平均升高，试验组高于对照组，差异均有统计学意义（均P＜0.05）。结论：加味补肺汤联合针灸疗法可明显改善系统性硬皮病患者中医证候、皮肤硬度、关节痛及关节功能，提高美学修复效果，并可改善患者免疫功能，进而有助于提高疗效。
Abstract:
Objective To investigate the efficacy of Jiawei Bufei decoction combined with acupuncture therapy in treating systemic scleroderma of deficiency of lung qi and its influence on serum immune factors. Methods A total of 80 patients with systemic scleroderma who were admitted to the dermatology department of Qianjiang central hospital from March 2017 to March 2020 as the research object, and they were divided into the test group and the control group by random number table method, with 40 cases in each group. The control group was given penicillamine tablets, vitamin B6 tablets, and vitamin B1 tablets for treatment. The test group was given Jiawei Bufei decoction combined with acupuncture therapy on the basis of the control group. Both groups were treated for 24 weeks. The efficacy after 24 weeks of treatment, the scores of traditional chinese medicine syndromes, skin hardness, joint pain, joint function and immune function before and after 24 weeks of treatment were compared of the two groups. Results After 24 weeks of treatment, the total effective rate in the test group was 95.00%, which was higher than 72.50% in the control group (P＜0.05). Compared with before treatment, after 24 weeks of treatment, the scores of thin white tongue coating, fatigue, fatigue, cold limbs, backache, weakness, total score of traditional chinese medicine syndromes, skin hardness, joint pain, joint function and the level of peripheral blood CD8 + decreased, the test group was lower than the control group (P ＜0.05). while the levels of peripheral blood CD4+, CD4+/CD8+ increased, the test group was higher than the control group (P ＜0.05). Conclusion Jiawei Bufei decoction combined with acupuncture therapy could significantly improve traditional chinese medicine syndromes, skin hardness, joint pain and joint function in patients with systemic scleroderma, improve aesthetic repair effects, and improve the immune function of patients, thereby helping to improve the efficacy
