摘要:

目的：探讨烤瓷冠分别联合活动义齿和固定义齿对牙列缺损患者咬合功能、脑氧供及炎症反应的影响。方法：选取2017年7月-2019年7月笔者医院就诊的122例牙列缺损患者，按随机数字表法分为两组，每组61例。两组均给予烤瓷冠，A组采用活动义齿，B组采用固定义齿。观察两组修复后6个月的修复效果、咬合功能、修复满意度与修复前、修复后3个月及6个月咀嚼效率、龈沟出血指数（SBI）、牙龈指数（GI）、血清炎症因子[白细胞介素-8（IL-8）、淀粉蛋白A（SAA）]，安静及咀嚼状态下大脑中动脉平均峰流速值（Vm）。结果：修复后6个月，A组修复总有效率93.44%、咬合优良率91.80%，高于B组的77.05%、75.41%，差异有统计学意义（P＜0.05）；修复后3、6个月，两组咀嚼效率均升高，SBI、GI降低，且A组咀嚼效率高于B组，SBI、GI低于B组，差异有统计学意义（P＜0.05）；修复后3、6个月，两组血清IL-8、SAA水平均降低，且A组低于B组，差异有统计学意义（P＜0.05）；修复后3、6个月两组安静及咀嚼状态下大脑中动脉Vm均升高，且A组高于B组，差异有统计学意义（P＜0.05）；修复后6个月，A组咀嚼功能、语言功能、舒适度、固位功能、外观修复满意度高于B组，差异有统计学意义（P＜0.05）。结论：与烤瓷冠联合固定义齿相比，烤瓷冠联合活动义齿治疗牙列缺损修复效果显著，可改善咀嚼效率、咬合功能，增加大脑中动脉血流速度，且对牙周组织损伤小，减轻炎症反应，修复满意度高。

Abstract:

Objective To investigate the effect of porcelain crown combined with movable denture and fixed denture on occlusion function, cerebral oxygen supply and inflammatory reaction in patients with dentition defects. Methods From July 2017 to July 2019, 122 patients with dentition defects in our hospital were selected and grouped by random number table method, 61 cases in each. Both groups were given porcelain crowns, group A was given removable dentures, and group B was given fixed dentures. The repair efficiency, bite function at 6 months after repair, repair satisfaction and chewing efficiency, sulcus bleeding index (SBI), gingival index (GI), serum inflammatory factors [interleukin-8 (IL-8), amyloid A (SAA)], the average peak velocity value of middle cerebral artery (Vm) in the quiet and chewing state before, 3 and 6 months after repair of the two groups were observed. Results 6 months after repair, the total effective rate of repair in group A was 93.44%, and the excellent and good rate of occlusion was 91.80%, which was higher than 77.05% and 75.41% in group B (P ＜0.05). The mastication efficiency of the two groups increased at 3 and 6 months after the repair, and the SBI and GI decreased. The mastication efficiency of group A was higher than that of group B, and SBI and GI were lower than group B (P＜0.05). Serum IL-8 and SAA levels in the two groups decreased at 3 and 6 months after repair, and group A was lower than group B (P＜0.05). At 3 and 6 months after the repair, the Vm of the middle cerebral artery in the two groups increased in the quiet and chewing state, and the A group was higher than the B group (P＜0.05). 6 months after the restoration, the satisfaction with masticatory function, language function, comfort, retention function, and appearance restoration in group A was higher than that in group B (P＜0.05). Conclusion Compared with porcelain crowns combined with fixed teeth, porcelain crowns combined with removable dentures have a significant effect in the treatment of dentition defects. It can improve chewing efficiency, occlusal function, increase the blood flow speed of the middle cerebral artery, and cause little damage to periodontal tissue, reduce inflammation, and have high repair satisfaction.
