摘要:
目的：探究美容缝合联合重组牛碱性成纤维细胞生长因子（Recombinant bovine basic fibroblast growth factor，rb-bFGF）外用治疗颌面部外伤的疗效。方法：收集2019年7月-2021年4月笔者医院收治的60例颌面外伤患者作为研究对象，根据随机数字表法分为对照组（n=30，传统缝合）和观察组（n=30，美容缝合联合rb-bFGF），对比两组患者的美容疗效、缝合时间、缝合费用、愈合时间、创面愈合情况（创口对位不齐、创面崩裂、创口感染）、瘢痕大小、炎症因子[白介素-1（Interleukin-1,IL-1）、白介素-6（Interleukin-6，IL-6）、肿瘤坏死因子-α（Tumor necrosis factor-ɑ, TNF-ɑ）]及血清表皮生长因子；采用温哥华评分量表（Vancouver rating scale, VSS）和双向瘢痕评分量表（Patient and observer scar assessment scale,POSAS）评估患者术后3个月、6个月瘢痕恢复程度及严重情况。结果：观察组治疗有效率高于对照组，差异具有统计学意义（χ2=4.043，P=0.044＜0.05）；观察组缝合时间长于对照组，缝合费用、愈合时间短于对照组，瘢痕大小较对照组小，术后瘢痕评分高于对照组，创面愈合情况优于对照组，差异均有统计学意义（P＜0.05）。术后3 d，两组患者IL-1、IL-6、TNF-ɑ均较术前下降，血清表皮生长因子较术前提高，观察组IL-1、IL-6、TNF-ɑ低于对照组，血清表皮生长因子高于对照组（P＜0.05）；两组患者术后3个月、6个月VSS、POSAS评分均明显降低，且观察组患者术后3个月、6个月VSS、POSAS评分均低于对照组，差异均有统计学意义（P＜0.05）。结论：颌面部外伤患者采用美容缝合联合rb-bFGF治疗，可提高美容效果和创口修复质量，缩短创口愈合时间，降低瘢痕增生，值得临床推广应用。
Abstract:
Objective To explore the efficacy of cosmetic suture combined with recombinant bovine basic fibroblast growth factor (rbFGF) in the treatment of maxillofacial trauma. Methods A total of 60 maxillofacial trauma patients admitted to our hospital from July 2019 to April 2021 were collected as the research objects. According to random number table method, they were divided into control group (n=30, traditional suture) and observation group (n=30, cosmetic suture combined with rbFGF). Aesthetic efficacy, suture time, suture cost, healing time, wound healing status (wound dislocations, wound disintegrating, wound infection), scar size, inflammatory factors [IL-1, IL-6, tumor necrosis factor-ɑ (TNF-ɑ)] and serum epidermal growth factor were compared between 2 groups. Using the Vancouver Rating Scale (VSS)And bidirectional Scar Score Scale (POSAS) to evaluate the degree and severity of scar recovery 3 and 6 months after surgery. Results The effective rate of observation group was higher than control group, the difference was statistically significant (χ2 =4.043, P=0.044＜0.05). The observation group had longer suture time, shorter suture cost and healing time than the control group, smaller scar size, higher postoperative scar score and better wound healing than the control group (P＜0.05). 3D after surgery, IL-1, IL-6, TNF-ɑ were decreased, and serum epidermal growth factor (EGF) was increased in 2 groups compared with before surgery, il-1, IL-6, TNF-ɑ were lower in observation group than control group, and serum EGF was higher than control group (P＜0.05). VSS and POSAS scores in 2 groups were significantly decreased 3 and 6 months after surgery (P＜0.05). VSS and POSAS scores in observation group were lower than those in control group at 3 and 6 months after surgery (P＜0.05). Conclusion For maxillofacial trauma patients, cosmetic suture combined with rbFGF therapy can improve cosmetic effect and wound repair quality, shorten wound healing time and reduce scar hyperplasia, which is worthy of clinical promotion.
