摘要:

目的：分析安氏Ⅱ类1分类错牙合不同直丝弓矫治器正畸治疗的头影测量指标及侧貌美学效果。方法：纳入2018年1 月-2020年6月笔者医院收治的80例安氏Ⅱ类1分类错牙合患者，按照随机数字表法分为MBT组与Z2组，每组40例，分别采用MBT 直丝弓矫治器与Z2直丝弓矫治器进行正畸治疗1.5年。比较两组治疗前后的头影测量指标(SNA、SNB、ANB、∠U1-SN、∠L1- MP、U1-PP、U6M-PP)、侧貌美学指标(H角、面型角、鼻唇角、颏沟倾角、上唇长、下唇长、上唇厚、下唇厚)及患者侧貌美 观满意率。结果：与治疗前比较，治疗后两组的SNA、SNB无明显变化(P＞0.05)，∠ANB、∠U1-SN、L1-MP、U1-PP、U6M-PP 明显降低(P＜0.05)；两组的下唇长、下唇厚无明显变化(P＞0.05)，H角明显降低(P＜0.05)，面型角、鼻唇角、颏沟倾角、 上唇长、上唇厚均明显增大(P＜0.05)；两组之间各项指标比较差异无统计学意义(P＞0.05)；MBT组与Z2组的患者侧貌美观 满意率分别为95.00%、92.50%，差异无统计学意义(P＞0.05)。结论：使用MBT与Z2直丝弓矫治器进行正畸治疗均能有效改善 安氏Ⅱ类1分类错牙合患者的口颌系统及侧貌美观，效果相当。

Abstract:

Objective To analyze the cephalometric indexes and lateral aesthetic eff ect of diff erent straight-wire appliance for orthodontic treatment of Angle Ⅱ class 1 malocclusion. Methods A total of 80 patients with Angle Ⅱ class 1 malocclusion who was included in the author ’s hospital from January 2018 to June 2020, and they were divided into MBT group and Z2 group according to random number table method, with 40 cases in each group. The patients were given MBT straight-wire appliance and Z2 straight-wire appliance were used for orthodontic treatment for 1.5 years.The cephalometric indexes( SNA, SNB, ANB, ∠U1-SN,∠L1-MP, UO-PP, U6M-PP), lateral aesthetic indexes(H angle, G-Sn-Pos, naso-labial angle, Pg’B’-FH, upper lip length, lower lip length, upper lip thickness, lower lip thickness) and patients’ satisfaction of profi le esthetics were compared between two groups before and after treatment. Results Compared with before treatment, the SNA and the SNB were not signifi cantly changed in two groups after treatment(P ＞0.05), and the ANB,∠U1-SN,∠L1-MP, UO-PP, U6M-PP were signifi cantly decreased in two groups after treatment(P ＜0.05). The lower lip length, lower lip thicknesswas not signifi cantly changed in two groups after treatment(P＞0.05), the H angle was significantly decreased in two groups after treatment(P ＜0.05), and the G-Sn-Pos, naso-labial angle, Pg’B’-FH, upper lip length, upper lip thickness were signifi cantly increased in two groups after treatment(P ＜0.05). There were no signifi cant diff erence on various indexes between two groups(P ＞0.05). The satisfaction rates of patients’ lateral aesthetic appearance in the MBT and Z2 groups were 95.00% and 92.50%, with no statistically signifi cant diff erence (P ＞0.05). Conclusion The two orthodontic treatment methods could eff ectively improve the stomatognathic system and lateral aesthetics of patients with Angle Ⅱ Class 1 malocclusion, and the eff ect was similar.
