摘要:
目的：探讨前牙美学牙冠延长术在上颌前牙牙体缺损修复中的应用及美学效果。方法：选取笔者科室2019年1月-2020 年6月收治的80例上颌前牙牙体缺损修复患者，按照随机数字表法分为A组和B组，每组40例。B组给予牙冠延长术进行修复， A 组给予常规前牙修复治疗。记录并对比两组患者牙菌斑指数(Plague index,PLI)、探诊深度(Probing depth,PD)、出血指 数(Bleeding index,BI)、咀嚼效率和咬合能力、美观程度以及患者满意度。结果：治疗后，两组患者PLI、BI比较，B组低 于A组，两组患者PD比较，B组高于A组（P＜0.05）；治疗后，B组的咀嚼效率、咬合能力均高于A组（P＜0.05），B组所有患 者均达到了Kay牙齿美学术后分类Class Ⅱ-Ⅰ或Class Ⅱ-Ⅰ（化妆）标准，且修复后美观效果患者满意。治疗后，B组的固 位功能、咀嚼功能、语言功能、美观度、舒适度评分均高于治疗前，差异有统计学意义（P＜0.05）。结论：前牙美学牙冠 延长术用于修复上颌前牙牙体缺损能够改善患者牙周指数，提高其咬合能力和咀嚼功能，符合美学要求，患者满意度高。
Abstract:
Objective To investigate the application and aesthetic eff ect of anterior crown lengthening in the repair of maxillary anterior tooth defect. Methods A selection of 80 patients with maxillary anterior tooth defect repaired in the department of stomatology of the author’s hospital from January 2019 to June 2020 were divided into group B and group A according to the random number table, with 40 cases in each group. Group B was given crown lengthening surgery for restoration, and group A was given conventional anterior tooth restoration treatment. Plaque index (PLI), probing depth (PD) and bleeding index (BI), masticatory effi ciency and occlusal ability, aesthetic degree before and after treatment and patient satisfaction were recorded and compared between the two groups. Results After treatment, the PLI and Bi of group B were lower than those of group A. The PD of group B was higher than that of group A (P ＜0.05). The masticatory effi ciency and occlusal ability of group B were higher than those of group A (P ＜0.05). All patients in group B met the standard of Kay dental aesthetics academic post classifi cation Class Ⅱ-Ⅰ or Class Ⅱ-Ⅰ(makeup), And the aesthetic eff ect after repair has reached the satisfaction of patients. After treatment, the scores of retention function, chewing function, language function, aesthetics and comfort in group B were higher than those before treatment (P＜0.05). Conclusion Anterior crown lengthening can improve periodontal index, occlusal ability and masticatory function, meet the aesthetic requirements and achieve the satisfaction of patients.
