摘要:
[摘要]目的：探究无托槽隐形矫治与传统直丝弓矫治对牙周炎正畸患者牙周健康状态及美观效果的影响。方法：将笔者医
院就诊的90例牙周炎正畸患者随机分为观察组（n =46）和对照组（n =44）。两组患者均给予牙周炎基础治疗，对照组在完
成基础治疗后予以传统直丝弓矫治器治疗，观察组予以无托槽隐形矫治。比较两组治疗前及治疗6个月后的面部硬组织指标
[前颅底与上颌基骨夹角(SNA)、前颅底与下颌基骨夹角(SNB)、上下基骨间夹角(ANB)、上齿槽座点与上中切牙点至鼻根点
连线的距离(U1-NA)、鼻根点下齿槽座点与下中切牙长轴连线的距离(L1-NB)、上下中切牙角(U1-L1)]、龈沟液标本中的细胞
因子[白细胞介素-6（IL-6）、人基质金属蛋白酶-8（MMP-8）、细胞核因子κB受体活化子配体(RANKL)]水平、牙周健康状
态指标[菌斑指数（PLI）、龈沟出血指数（SBI）、牙龈指数（GI）、牙周袋探诊深度（PD）、临床附着丧失（CAL）]变化
及不同时间点的美观效果[正畸治疗需要指数美学成分量表(IOTN-AC量表)]。结果：治疗前，两组SNA、SNB、ANB、U1-NA、
L1-NB、U1-L1比较无统计学意义（P ＞0.05），治疗6个月后，两组SNA、SNB、ANB、U1-NA、L1-NB、U1-L1与治疗前比较无统
计学意义（P ＞0.05），且组间比较无统计学意义（P ＞0.05）；治疗6个月后，两组患者龈沟液IL-6、MMP-8、RANKL水平及
PLI、SBI、GI、PD、CAL较治疗前均有显著上升（P ＜0.05），观察组明显低于对照组（P ＜0.05）；观察组患者治疗6、12个
月后的IOTN-AC量表评分均明显低于对照组（均P ＜0.05）。结论：在牙周炎正畸患者的口腔矫治过程中，无托槽隐形矫治器
较直丝弓矫治器对患者炎症因子的控制更佳，有利于患者牙周状态的改善，且美观效果更好。

Abstract:
Abstract: Objective To explore the influence of bracketless invisible orthodontics and traditional straight wire orthodontics on
periodontal health status and aesthetic effects of orthodontic patients with periodontitis. Methods A total of 90 patients with
orthodontic periodontitis in our hospital were randomly divided into observation group (n =46) and control group (n =44).Both
groups were given basic treatment of periodontitis,and control group was treated with traditional straight wire appliance after
the basic treatment,and observation group was treated with bracketless invisible orthodontics. The facial hard tissue indicators
[angle between anterior skull base and maxillary base bone (SNA), angle between anterior skull base and mandibular base
bone (SNB), angle between maxillary and mandibular base bones (ANB),distance between subspinale and upper central incisor
point to the nasal root point connection (U1-NA), distance between subspinale of the nasal root point and the long axis of lower
central incisor (L1-NB),upper and lower central incisor angle (U1-L1)],and the cytokines [interleukin-6 (IL-6),human matrix
metalloproteinase-8 (MMP-8),receptor activator nuclear factor κB ligand (RANKL)] and periodontal health indicators [plaque
index (PLI),sulcus bleeding index (SBI), gingival index (GI),periodontal probing depth (PD), clinical attachment loss (CAL)]
in gingival crevicular fluid specimens and the aesthetic effects at different time points [Aesthetic Component of the Index of Orthodontic
Treatment Need (IOTN-AC Scale)] were compared between the two groups before treatment and after 6 months of treatment. Results
Before treatment,there were no significant differences in SNA, SNB, ANB, U1-NA, L1-NB and U1-L1 between the two groups (P＞
0.05). After 6 months of treatment, there were no significant differences in SNA, SNB, ANB,U1-NA,L1-NB and U1-L1 in the
two groups compared with those before treatment (P ＞0.05),and there were no significant differences between the two
groups (P ＞0.05).After 6 months of treatment, the levels of IL-6, MMP-8 and RANKL and PLI,SBI, GI,PD and CAL in
gingival crevicular fluid of the two groups were significantly higher than those before treatment (P ＜0.05),and the above
indicators in observation group were significantly lower than those in control group (P ＜0.05).The scores of IOTN-AC scale
in observation group at 6 and 12 months after treatment were significantly lower than those in control group (all P ＜0.05).
Conclusion In the process of orthodontics,bracketless invisible appliance can better control inflammatory factor than straight
wire appliance,and it is beneficial to the improvement of periodontal status, with better aesthetic effects.
