目的：研究外周血IFN-γ、IL-4及IFN-γ/IL-4比值与急性湿疹皮损程度的相关性，并分析其预测急性湿疹短期疗效的临床价值。方法：回顾性分析笔者医院126例急性湿疹患者的临床资料，根据湿疹面积及严重度指数（EASI）评分将患者分为＜10分组（n=36）、10～20分组（n=66）、＞20分组（n=24）。观察患者短期治疗效果，根据疗效将患者分为有效组（n=24）和无效组（n=102）。收集患者一般资料及IFN-γ、IL-4、IFN-γ/IL-4水平，分析外周血IFN-γ、IL-4及IFN-γ/IL-4比值与急性湿疹皮损程度的关系及对短期疗效的预测价值。结果：EASI评分＞20分组IFN-γ、IFN-γ/IL-4水平显著低于EASI评分＜10分和10～20分组，差异有统计学意义（P＜0.05），IL-4水平显著高于EASI评分＜10分和10～20分组（P＜0.05）；EASI评分10～20分组IFN-γ、IFN-γ/IL-4水平显著低于EASI评分＜10分组（P＜0.05），IL-4水平显著高于EASI评分＜10分组（P＜0.05）。急性湿疹患者EASI评分与外周血IFN-γ及IFN-γ/IL-4比值呈负相关（P＜0.05），与IL-4呈正相关（P＜0.05）。无效组IL-4水平及焦虑患者占比均显著高于有效组（P＜0.05），IFN-γ、IFN-γ/IL-4水平均显著低于有效组（P＜0.05）。多因素Logistic回归分析显示，焦虑、IFN-γ、IL-4、IFN-γ/IL-4是急性湿疹短期疗效的独立影响因素（P＜0.05）。IFN-γ、IL-4及IFN-γ/IL-4联合预测急性湿疹短期疗效的灵敏度为0.921、特异度0.833。结论：外周血IFN-γ、IL-4及IFN-γ/IL-4比值与急性湿疹皮损程度存在显著相关性，且对患者短期疗效具有一定预测价值。
Abstract:
Objective Study peripheral blood IFN- γ, IL-4 and IFN-γ/IL-4 ratio and the degree of skin lesions in acute eczema, and its clinical value in predicting the short-term efficacy of acute eczema was analyzed. Methods The clinical data of 126 patients with acute eczema in the author’s hospital were retrospectively analyzed. According to the Eczema Area and Severity Index (EASI) score, patients were divided into ＜10 groups (n=36), 10 to 20 groups (n=66), and ＞20 groups (n=24). Observe the short-term treatment effect of patients, and divide the patients into effective group (n=24) and ineffective group (n=102) according to the curative effect. Collect patients’ general data and levels of IFN-γ, IL-4, IFN-γ/IL-4, analyze the relationship between peripheral blood IFN-γ, IL-4 and IFN-γ/IL-4 ratios and the extent of acute eczema skin lesions and The predictive value of short-term efficacy. Results The levels of IFN- γ and IFN- γ/IL-4 in the EASI score>20 group were significantly lower than those in the EASI score<10 points and the 10-20 groups, the difference was statistically significant (P＜0.05), and the IL-4 level was significantly higher than EASI Score ＜10 points and 10-20 group (P＜0.05), EASI score 10-20 group IFN-γ, IFN-γ/IL-4 levels were significantly lower than EASI score＜10 group (P＜0.05), IL-4 The level was significantly higher than the EASI score<10 group (P＜0.05). The EASI score of patients with acute eczema was negatively correlated with peripheral blood IFN-γ and IFN- γ/IL-4 ratio (P＜0.05), and positively correlated with IL-4 (P＜0.05). The levels of IL-4 and the proportion of anxiety patients in the ineffective group were significantly higher than those in the effective group (P＜0.05), and the levels of IFN-γ and IFN- γ/IL-4 were significantly lower than those in the effective group (P＜0.05). Multivariate logistic regressionanalysis showed that anxiety, IFN-γ, IL-4, IFN-γ/IL-4 were independent factors influencing the short-term efficacy of acute eczema (P＜0.05). The sensitivity of IFN-γ, IL-4 and IFN-γ/IL-4 combined to predict the short-term curative effect of acute eczema is 0.921 and the specificity is 0.833. Conclusion Peripheral blood IFN- γ, IL-4 and IFN-γ/IL-4 ratio were significant correlation with the degree of acute eczema lesions, and has a certain predictive value for the short-term efficacy of patients
