摘要:

目的：探讨氧化锆全瓷冠与纯钛烤瓷全冠修复牙列缺损患者的美学效果及咀嚼功能、语言能力的差异性。方法：选取 2018 年12月-2021年3月在唐山市协和医院口腔科就诊的106例牙列缺损患者作为研究对象，按随机数字表法将患者分为氧化 锆全瓷冠组和纯钛烤瓷全冠组，每组53例。氧化硅全瓷冠组采用氧化硅全瓷冠修复，纯钛烤瓷全冠组采用纯钛烤瓷全冠修 复。对两组的临床疗效进行评价，比较两组患者的咀嚼效率和义齿就位率，对两组患者的舒适度、语言能力、咀嚼力度和美 观度评分进行评估，采用金刚探针牙周诊断仪检测两组患者的牙周袋深度，根据菌斑量和厚度积分对患者的牙齿菌斑指数进 行评价。观察并记录两组患者修复后基托受损、基托不良、牙周病等不良反应发生情况。结果：氧化锆全瓷冠组的临床疗效 优良率90.57%，高于纯钛烤瓷全冠组的69.81%（χ2 =7.185，P＜0.05）；氧化锆全瓷冠组的咀嚼效率（87.22±3.16）%，高 于纯钛烤瓷全冠组的（76.22±2.36）%（t=20.305，P＜0.05），就位率（96.23%）高于纯钛烤瓷全冠组（83.02%），差异 有统计学意义（χ2 =4.970，P＜0.05）；氧化锆全瓷冠组舒适度、语言能力、咀嚼力度和美观度评分均高于纯钛烤瓷全冠组 （t=3.019、3.143、3.308、3.042，P＜0.05）；氧化锆全瓷冠组牙周袋深度和牙齿菌斑指数均低于纯钛烤瓷全冠组，差异 均有统计学意义（t=5.590、6.951，P＜0.05）；氧化锆全瓷冠组基托受损、基托不良、牙周病发生率均低于纯钛烤瓷全冠 组，差异均有统计学意义（χ2 =4.867、4.711、7.361，P＜0.05）。结论：氧化锆全瓷冠修复可有效提升修复效果，改善牙 列缺损患者的咀嚼功能，提高其舒适度、语言能力、咀嚼功能以及美观效果，降低不良反应发生率，牙周病防治效果较佳。

Abstract:

Objective To investigate the difference of aesthetic effect, masticatory function and language ability between zirconia all ceramic crown and pure titanium porcelain crown in the treatment of dentition defect. Methods: 106 patients with dentition defect in our hospital from December 2018 to March 2021 were selected and randomly divided into zirconia all ceramic crown group and pure titanium ceramic crown group, 53 cases in each group. The clinical efficacy of the two groups were judged, and the masticatory effi ciency and denture placement rate of the two groups were compared. The comfort, language ability, masticatory strength and aesthetic eff ect of the two groups were evaluated. The pocket depth of the two groups was detected by diamond probe periodontal diagnostic instrument, and the dental plaque index of the two groups was evaluated according to the plaque amount and thickness score. The incidence of adverse reactions such as damaged base, bad base and periodontal disease were observed and recorded. Results The excellent and good clinical eff ect rate of zirconia all ceramic crown group was 90.57%, which was higher than that of pure titanium porcelain crown group 69.81% (χ2 =7.185 ， P ＜0.05).The masticatory effi ciency of zirconia all ceramic crown group was (87.22±3.16)%, which was higher than that of pure titanium PFM crown group (76.22±2.36)% (t=20.305 ，P＜0.05), and the placement rate of 96.23% was higher than that of 83.02%(χ2 =4.970，P＜0.05）. The scores of comfort, language ability, masticatory strength and aesthetics in the zirconia all ceramic crown group were higher than those in the pure titanium ceramic crown group (t=3.019, 3.143, 3.308, 3.042, P＜0.05). The pocket depth and dental plaque index of zirconia all ceramic crown group were lower than those of pure titanium all ceramic crown group,the diff erences were statistically signifi cant （t=5.590，6.951，P＜0.05）. The incidence of base damage, poor base and periodontal disease in zirconia all ceramic crown group was lower than that in pure titanium all ceramic crown group, the diff erences were statistically signifi cant （χ2 =4.867, 4.711, 7.361, P＜0.05）. Conclusion Zirconia all ceramic crown restoration can eff ectively improve the eff ect of restoration, the masticatory function of patients with dentition defect, their comfort level, the language ability, masticatory function and aesthetic eff ect. They can reduce the incidence of base damage, bad base and adverse reactions of periodontal disease, periodontal disease prevention and treatment eff ect is better.
