摘要:
目的:观察强脉冲光联合氨甲环酸凝胶贴治疗黄褐斑的效果。方法:选取2020年6月-2021年6月笔者医院收治的80例 黄褐斑患者作为研究对象,经随机数字表法分为观察组和对照组各40例。对照组患者予以强脉冲光治疗,观察组在对照组基 础上加用氨甲环酸凝胶贴。比较两组患者治疗前、治疗2个月后临床效果[黄褐斑面积及严重程度评分(MASI)、皮损颜色、 皮损面积]、皮肤状态(VISIA皮肤图像分析)、皮肤生理参数[经皮水分丢失(TEWL)、表皮油脂含量、角质层含水量]、皮 肤菌群(微球菌、需氧革兰阴性杆菌、痤疮丙酸杆菌)水平,统计两组患者治疗2个月内不良反应发生情况。结果:治疗2 个月后,两组患者MASI、皮损颜色、皮损面积、VISIA皮肤图像分析结果(色素沉着、色斑、毛孔、纹理水平)、TEWL水 平显著低于治疗前,且除TEWL外,观察组其余指标均显著低于对照组(P<0.05),两组患者TEWL水平比较差异无统计学 意义(P>0.05);两组患者角质层含水量显著高于治疗前(P<0.05),两组比较差异无统计学意义(P>0.05);观察组 患者皮肤表面微球菌、痤疮丙酸杆菌水平显著低于治疗前,且微球菌水平显著低于对照组(P<0.05),痤疮丙酸杆菌水平 两组比较差异无统计学意义(P>0.05);观察组患者表皮油脂含量显著高于治疗前,且显著高于对照组(P<0.05),对照 组与治疗前差异无统计学意义(P>0.05);两组患者需氧革兰阴性杆菌水平与治疗前比较差异无统计学意义(P>0.05)。 治疗2个月内,两组患者不良反应发生情况比较差异无统计学意义(P>0.05)。结论:强脉冲光联合氨甲环酸凝胶贴治疗黄 褐斑患者临床效果较好,可有效改善皮损,修复皮肤屏障,且安全性较好。
Abstract:
Objective To observe the eff ects of intense pulsed light combined with tranexamic acid gel patch in the treatment of chloasma. Methods 80 patients with chloasma who were treated in the hospital between June 2020 and June 2021 were selected as the research subjects and were divided into an observation group (40 cases) and a control group (40 cases) by the random number table method. The patients in the control group were treated with intense pulsed light, and the patients in the observation group were additionally given tranexamic acid gel patches based on the control group. The clinical eff ects [Melasma Area and Severity Index (MASI), skin lesion color, skin lesion area], skin status (VISIA skin image analysis), skin physiological parameters [trans epidermal water loss (TEWL), epidermal grease content, water content of stratum corneum] and skin fl ora (Micrococcus, aerobic Gram-negative bacilli, propionibacterium acnes) were compared between the two groups before treatment and after 2 months of treatment. The occurrence of adverse reactions in the two groups of patients was statistically analyzed within 2 months of treatment. Results After 2 months of treatment, the MASI, skin lesion color, skin lesion area, VISIA skin image analysis results (pigmentation, stains, pores, texture) and TEWL in the two groups were signifi cantly lower than those before treatment, and the other indicators except for TEWL in the observation group were signifi cantly lower than those in the control group (P <0.05), and there was no signifi cant diff erence in TEWL between the two groups (P >0.05). The water content of stratum corneum in both groups was signifi cantly more than that before treatment (P <0.05), but there was no signifi cant diff erence between the groups (P >0.05). The levels of Micrococcus and Propionibacterium acnes on the skin surface were signifi cantly lower in the observation group than those before treatment, and the level of Micrococcus was signifi cantly lower than that in the control group (all P <0.05), but the diff erence in the level of Propionibacterium acnes was not signifi cant between the two groups (P>0.05). The content of epidermal grease in the observation group was signifi cantly more than that before treatment and was signifi cantly more than that in the control group (P <0.05), but the diff erence was not signifi cant in the control group compared to before treatment(P >0.05). The level of aerobic Gram-negative bacilli in the two groups was not significantly different from that before treatment, and there was no significant difference between the two groups (P>0.05). Within 2 months of treatment, there were no statistical diff erences in the occurrence of adverse reactions between the two groups (P >0.05). Conclusion The combination of intense pulsed light and tranexamic acid gel patch has good clinical eff ects on patients with chloasma, and can eff ectively improve the skin lesions and repair the skin barriers, with good safety.
