[摘要]目的:探讨脉冲染料激光(PDL)联合CO2点阵激光治疗烧伤后增生性瘢痕的临床疗效。方法:选取2018年6月-2019年
8月笔者医院收治的96例烧伤后增生性瘢痕患者为研究对象,随机分为观察组和对照组,每组48例。对照组采用CO2点阵激
光治疗,观察组采用脉冲染料激光联合CO2点阵激光治疗,比较两组患者治疗6个月后的总有效率,治疗前后瘢痕厚度、瘢
痕面积、温哥华瘢痕量表(VSS)评分、疼痛视觉模拟评分(VAS)、瘙痒数字评分(NRS),以及治疗期间不良反应发生
率。结果:治疗后,观察组患者的总有效率明显高于对照组(P <0.05);观察组患者的瘢痕厚度、瘢痕面积、疼痛VAS评
分、瘙痒NRS、VSS各项评分和总分均低于对照组(P <0.05);且两组患者在治疗期间的不良反应发生率比较无显著性差
异(P >0.05)。结论:CO2点阵激光联合PDL治疗烧伤后增生性瘢痕可早期抑制血管过度增生,改善瘢痕大小、皮肤外观和
疼痛瘙痒症状,提高临床疗效。

Abstract:
Abstract:Objective To explore clinical curative effect of pulsed dye laser (PDL) combined with CO2 fractional laser on
hypertrophic scar after burn. Methods A total of 96 patients with hypertrophic scars after burn who were admitted to the
hospital from June 2018 to August 2019 were enrolled as study objects. They were randomly divided into observation group
and control group, 48 cases in each group. The control group was treated with CO2 fractional laser, while observation group
was treated with PDL combined with CO2 fractional laser. The total response rate after 6 months of treatment, scar thickness,
scar area, scores of Vancouver Scar Scale (VSS), pain visual analogue scale (VAS) and numeric rating scale (NRS) before and
after treatment, and incidence of adverse reactions during treatment were compared between the two groups. Results After
treatment, total response rate in observation group was significantly higher than that in control group (P <0.05). The scar
thickness, scar area, scores of pain VAS, pruritus NRS and VSS, and their total scores in observation group were lower than
those in control group (P <0.05). There was no significant difference in incidence of adverse reactions during treatment period
between the two groups (P >0.05). Conclusion CO2 fractional laser combined with PDL in treatment of hypertrophic scars
after burn can early inhibit vascular hyperplasy, improve scar size, skin appearance and symptoms such as pain and itching, and
improve clinical curative effect.
