摘要:
目的:探讨强脉冲光(Intense pulsed light,LPL)联合酵母重组胶原蛋白凝胶治疗面部糖皮质激素依赖性皮炎 (Facial hormone dependent dermatitis,FHDD)的效果。方法:将2021年1月-2022年3月笔者医院收治的100例FHDD患者 随机均分为观察组和对照组,每组50例。两组均采用IPL及其他常规措施治疗,观察组加涂酵母重组胶原蛋白凝胶治疗,对 照组加涂安慰剂(酵母重组胶原蛋白模拟状凝胶)。比较治疗前后两组疗效、皮肤生理参数、血清炎症因子和免疫球蛋白 水平、皮肤病生活质量指数(Dermatology life quality index,DLQI)。结果:观察组治疗总有效率80.00%高于对照组 52.00% (P<0.05)。治疗后,两组经皮水分散失(Transepidermal water loss,TEWL)水平、血清白细胞介素(IL-4)、干扰 素(IFN-γ)、免疫球蛋白(IgE、IgA、IgM)水平、DLQI量表评分均较治疗前降低,且观察组低于对照组(P<0.05)。两组 患者表皮皮脂含量和角质层含水量较治疗前升高,且观察组高于对照组(P<0.05)。两组患者不良反应发生率比较差异无 统计学意义(P>0.05)。随访6个月,观察组复发率5.00%低于对照组25.00%(P<0.05)。结论:IPL联合酵母重组胶原蛋 白凝胶治疗FHDD效果确切,能改善皮肤生理参数,调节血清炎症因子和免疫球蛋白水平,提高患者生活质量。
Abstract:
Objective To investigate the effi cacy of intense pulsed light combined with yeast recombinant collagen gel in the treatment of facial hormone dependent dermatitis (FHDD). Methods Using a randomized, double-blind trial,100 FHDD patients admitted from January 2021 to March 2022 were randomly divided into observation group and control group, with 50 cases in each group.Both groups were treated with intense pulsed light and other routine measures. The observation group was treated with yeast recombinant collagen gel and the control group was treated with placebo (yeast recombinant collagen mimic gel ).The clinical effi cacy,skin physiological parameters, serum infl ammatory factors and immunoglobulin levels, and Dermatology Life Quality Index (DLQI) were compared between the two groups before and after treatment. Results The total excellence rate of 80.00% in the observation group was higher than 52.00% in the control group (P <0.05).After treatment, the levels of transepidermal water loss (TEWL), the levels of serum interleukin-4 (IL-4), interferon (IFN- γ), immunoglobulin ( IgE, IgA, IgM) and the score of DLQI scale in the two groups of patients were lower than those before treatment, and the observation group was lower than those of the control group(P<0.05).The skin epidermal sebum content and cuticle water content of the two groups were higher than that before treatment, and the observation group was higher than those of the control group (P<0.05).There was no signifi cant diff erence in the incidence of adverse reactions between the two groups (P >0.05).After a 6-month followup, The recurrencerate of 5.00% in the observation group was lower than 25.00% in the control group (P <0.05). Conclusion Intense pulsed light combined with yeast recombinant collagen gel is more effective than intense pulsed light alone in the treatment of FHDD, which can improve skin physiological parameters, regulate the levels of serum infl ammatory factors and immunoglobulins, and improve the quality of life of patients.
