摘要:
目的：分析固定-活动义齿在牙齿重度磨耗伴缺失治疗中的应用效果。方法：回顾性分析2020年2月-2021年10月在 笔者医院行口腔修复治疗的88例牙齿重度磨耗伴缺失患者的临床资料，按治疗方式的不同分为对照组（n=43，活动义齿修 复）和研究组（n=45，固定-活动义齿修复）。修复后半年判定两组修复效果，并比较两组患者修复前及修复后半年咀嚼 效能、颞下颌关节（Temporomandibular joint,TMJ）功能、生活质量及修复满意度。结果：研究组修复效果（95.56%）明 显高于对照组（81.40%）（P＜0.05）；修复后两组咬合力、咀嚼效率和修复前相比均明显增高（P＜0.05），且研究组高于 对照组（P＜0.05）；修复后两组Helkimo临床功能障碍指数、TMJ功能指数（Fricton指数）中的TMJ紊乱指数评分、口腔健 康影响程度量表14（Oral health impact scale 14,OHIP-14）评分和修复前相比均明显降低（P＜0.05），且研究组低于对 照组（P＜0.05）；研究组患者修复满意度评分高于对照组（P＜0.05）。结论：应用固定-活动义齿对牙齿重度磨耗伴缺失 患者进行修复的效果较佳，可明显改善患者咀嚼效能和TMJ功能，有效提高其生活质量，并能够提升患者满意度。?/html>
Abstract:
Objective To analyze the eff ect of fi xed movable denture in the treatment of severe tooth wear and loss. Methods The clinical data of 88 patients with severe tooth wear and loss who underwent oral prosthodontics in the author’s hospital from February 2020 to October 2021 were retrospectively analyzed, and they were grouped into control group (n=43, removable denture repair) and research group (n=45, fi xed combined removable denture repair). 6 months after the repair, the repair eff ect of the two groups was determined, and the masticatory efficacy, temporomandibular joint temporomandibular joint (TMJ) function, quality of life and repair satisfaction score were compared between the two groups before and after the repair. Results The repair eff ect of the research group was greatly higher than that of the control group (95.56% vs. 81.40%) (P ＜0.05). After repair, the biting force and chewing effi ciency of the two groups were greatly higher than those before repair (P ＜0.05), and the research group were greatly higher than the control group (P＜0.05). After repair, the Helkimo clinical dysfunction index,TMJ disorder index score in TMJ functional index ( fricton index) and oral health impact scale 14 (OHIP-14) score in the two groups were greatly lower than those before repair (P＜0.05), and the research group were greatly lower than the control group (P＜0.05), the repair satisfaction score in the research group were greatly higher than in the control group (P＜0.05). Conclusion The application of fi xed combined removable denture repair in the restoration of severe tooth wear and loss is eff ective, which can signifi cantly improve the masticatory effi ciency and TMJ function of patients, eff ectively improve their quality of life, and can improve the satisfaction of patients on treatment.
