摘要:
目的：研究全瓷冠和高嵌体修复上颌前磨牙缺损的美学效果以及对牙周状态的影响。方法：选取笔者医院2019年1 月-2022年1月收治的85例上颌前磨牙缺损患者作为研究对象，将患者随机分为对照组（n=42）和观察组（n=43）。对照组采 用全瓷冠修复方式，观察组采用高嵌体修复方式，比较两组患者的美学效果、牙周状态、咀嚼功能及并发症发生情况。结 果：①修复后6个月，两组黏膜质地和龈乳头得分均上升，且观察组明显高于对照组（P＜0.05）；与修复后6个月相比，修 复后12个月两组黏膜质地和龈乳头得分均上升，且观察组明显高于对照组（P＜0.05）。②与修复前相比，修复后6个月两组 探诊深度（Probing depth,PD）值和牙龈指数（Gingival index,GI）值均降低，且观察组略低于对照组（P＜0.05）；与修 复后6个月相比，修复后12个月两组PD值和GI值均降低（P＜0.05），且观察组明显低于对照组（P＜0.05）。③修复后6个 月，两组咀嚼效率和咬合力得分均上升（P＜0.05），且观察组高于对照组（P＜0.05）；与修复后6个月相比，修复后12个 月两组咀嚼效率和咬合力得分均下降，但观察组明显高于对照组（P＜0.05）。④修复后6个月，观察组患者继发龋和咬合痛 得分略低于对照组；与修复后6个月相比，修复后12个月观察组继发龋和咬合痛得分明显低于对照组（P＜0.05）。结论：对 于前磨牙缺损患者采用高嵌体修复相较全瓷冠修复美学效果及咀嚼功能更好，有利于牙周健康且并发症发生率更低，适合临 床推广使用。
Abstract:
Objective To study the aesthetic eff ect of all-ceramic crown and onlay in the repair of maxillary premolar defects and its infl uence on periodontal status. Methods 85 patients with maxillary premolar defects admitted to the hospital from January 2019 to January 2022 were selected as the study subjects, and were divided into control group (n=42) and observation group (n=43) by the randomization method. The control group received all-ceramic crown repair, and the observation group was given onlay repair. The aesthetic effect, periodontal status, masticatory function and occurrence of complications were compared between the two groups of patients. Results ①In terms of shape contour and dental crown color, the scores of aesthetic eff ect aspects in observation group were higher than those in control group at 6 months and 12 months after repair (P ＜0.05). Compared with before repair, the scores of mucosal texture and gingival papilla were increased at 6 months after repair, and the scores in observation group were slightly higher than those in control group during the same period (P＜0.05). The scores of mucosal texture and gingival papilla were enhanced at 12 months after repair compared with those at 6 months after repair, and the scores in observation group were signifi cantly higher than those in control group (P ＜0.05). ②The probing depth (PD) and gingival index (GI) were decreased at 6 months after repair compared to before repair (P＜0.05), and the two indicators in observation group were slightly lower compared with those in control group during the same period (P＜0.05). The PD and GI at 12 months after repair were reduced compared with those at 6 months after repair (P＜0.05), and the PD and GI were signifi cantly lower in observation group than those in control group (P ＜0.05). ③Compared with before repair, the scores of masticatory effi ciency and occlusal force were risen at 6 months after repair , and the scores in observation group were higher than those in control group (P ＜0.05). The scores of masticatory effi ciency and occlusal force at 12 months after repair were decreased compared to 6 months after repair, and the scores in observation group were signifi cantly higher compared to control group (P ＜0.05). ④At 6 months after repair, the scores of secondary caries and occlusal pain in observation group were slightly lower compared with those in control group (P＜0.05). Compared with 6 months after repair, the scores of secondary caries and occlusal pain in observation group at 12 months after repair were signifi cantly lower than those in control group (P ＜0.05). Conclusion Compared with all-ceramic crown repair, onlay repair has better aesthetic effect on patients with maxillary premolar defects, and it is more benefi cial to periodontal health and has better masticatory function and lower incidence rate of complications.
