摘要:
目的：探讨美容缝合技术在颌面部外伤清创处理中的应用效果。方法：选择2018年3月-2019年12月重庆市中医院皮肤美容科收治的颌面部外伤患者为研究对象，共106例，通过随机数字表法分为试验组和对照组，每组53例。对照组给予普通清创缝合术治疗，试验组给予美容缝合术治疗，两组术后均随访1个月。比较两组围术期手术指标、术后1个月的临床疗效，术前、术后3d、7d的疼痛程度，术前、术后3d的血清炎性因子水平及随访期间的并发症发生率。结果：试验组瘢痕长度、水肿持续及切口愈合时间均较对照组缩短，差异有统计学意义（P＜0.05）；术后1个月，试验组总有效率高于对照组（92.45% vs. 69.81%，P＜0.05），差异有统计学意义；与术前比较，术后3、7d，两组VAS评分均呈逐渐降低趋势，试验组低于对照组，差异有统计学意义（P＜0.05）。与术前比较，术后3d，两组血清IL-1、IL-6、TNF-α水平均降低，试验组低于对照组，差异有统计学意义（P＜0.05）。随访期间，试验组并发症发生率低于对照组（9.43%vs.24.53%），差异有统计学意义（P＜0.05）。结论：相比普通清创缝合术，美容缝合术可明显改善颌面部外伤患者手术指标，减轻疼痛程度及炎症反应，进而可提高临床疗效，且安全性较好。
Abstract:
Objective To investigate the application effect of cosmetic suture technique in the treatment of maxillofacial trauma. Methods A total of 106 patients with maxillofacial trauma admitted to the department of dermatology and aesthetics of Chongqing hospital of traditional Chinese medicine between March 2018 and December 2019 were selected as the research object. And they were divided into the test group and the control group by random number table, with 53 cases in each group. The control group was treated with ordinary debridement and suture, and the test group was treated with cosmetic suture. Both groups were followed up for 1 month. The perioperative surgical indicators, the clinical efficacy of 1 month after surgery, the pain level before and 3, 7 d after surgery, the serum inflammatory factors level before and 3 d after surgery, and the occurrence of complications rate during the followed up in the two groups were compared. Results The scar length, edema duration and incision healing time in the test group were shorter than those in the control group (P ＜0.05). 1 month after surgery, the total effective rate in the test group was higher than the control group (92.45% vs. 69.81%, P＜0.05). Compared with before surgery, 3, 7 d after surgery, the VAS scores in the two groups showed a gradually decreasing trend, and the test group was lower than the control group (P＜0.05). Compared with before surgery, 3 d after surgery, the serum IL-1, IL-6 and TNF-α levels in the two groups decreased, and the test group was lower than the control group (P ＜0.05).During followed up, the incidence of complications rate in the test group was lower than the control group (9.43% vs. 24.53%, P＜0.05). Conclusion Compared with ordinary debridement and suture, cosmetic suture could significantly improve the surgical indicators of maxillofacial trauma patients, and could reduce the body’s pain and inflammation, thereby helping to improve the clinical efficacy and with higher safety.
