摘要:
目的：探讨眶隔-提肌腱膜增厚区法（Septoaponeurosis junctional thickening，SAJT）在重 睑成形术中的临床应 用效果。方法：前瞻性纳入2020年1月-12月到笔者医院就诊的50例单睑就医者作为研究对象；切除就医者上睑的一条眼轮匝 肌将眶隔进行暴露，打开眶隔且处理脂肪后完全暴露SAJT，将SAJT与切口下唇的眼轮匝肌进行间断缝合，然后进行皮肤缝 合；于术后第6个月，采用温哥华瘢痕量表（Vancouver scar scale，VSS）评估上睑瘢痕情况，采用受试者整体审美改善量表 （Subjects global aesthetic improvement scale，S-GAIS）对术后美学效果进行评价；观察术后并发症情况。结果：50例 就医者的手术切口均为一期愈合，重睑线弧度及对称性较好，睁眼时重睑线流畅，闭眼时切口隐蔽平整；所有就医者的VSS评 分均为0分；45例（90.00%）就医者S-GAIS评价为非常满意，5例（10.00%）就医者S-GAIS评价为满意；无感染、重睑线变窄、 上睑下垂及瘢痕增生等术后并发症发生。结论：SAJT法在重睑成形术中的应用效果显著，美学效果好，无明显术后并发症。
Abstract:
Objective To investigate the clinical application effect of septoaponeurosis junctional thickening（SAJT）method on double eyelid plasty. Methods 50 patients with single eyelid who were prospectively included as the research subjects in our hospital from January to December 2020. The orbital septum was exposed by excision of one of the orbicularis oculi muscle of the patients upper eyelid. After the orbital septum was opened and the fat was treated, the SAJT was completely exposed. The SAJT was intersected with the orbicularis oculi muscle below the incision line, and then skin suture was performed. The Vancouver scar scale（VSS） was used to evaluate the upper eyelid scar, and the subjects global aesthetic improvement (S-GAIS） was used to evaluate the postoperative aesthetic effect.The postoperative complications were observed. Results The surgical incisions were all primary healing in 50 patients, with good radian and symmetry of double eyelid lines. The double eyelid lines were smooth when the eyes were opened, and the incisions were concealed and smooth when the eyes were closed. The VSS scores were all 0 points in 50 patients. The S-GAIS evaluation was very satisfactory in 45 patients（90.00%） and satisfactory in 5 patients（10.00%）. There were no postoperative complications such as infection, double eyelid line narrowing, ptosis and scar. Conclusion The SAJT method had significant application effect on double eyelid plasty, with good aesthetic effect and no obvious postoperative complications.
