摘要:
目的：探究前牙美学区不同牙龈生物型对上前牙全瓷冠修复患者牙齿美学效果的影响。方法：选取2019年4月-2021 年2月于笔者医院行上前牙全瓷冠修复的72例患者，根据牙周探诊结果将患者牙龈生物型分为薄龈组（n =29）与厚龈组 （n=43）。比较两组修复后1年的美观效果、牙龈指数、颜色满意度和牙龈边缘退缩程度。结果：修复后，两组软组织形态 评分比较，差异无统计学意义（P＞0.05），厚龈组软组织颜色、质地及红色美学指数评分(Pink ethetic score,PES)总分 高于薄龈组，差异有统计学意义（P＜0.05）。修复后，两组牙龈指数显著上升，且厚龈组牙龈指数高于薄龈组；薄龈组牙 龈边缘颜色满意度（Gingival margin color satisfaction，GMCS）评分下降，厚龈组上升，且厚龈组评分高于薄龈组，差 异均有统计学意义（P＜0.05）。修复前两组牙龈边缘均未退缩，修复后厚龈组牙龈边缘退缩程度小于薄龈组，差异有统计 学意义（P＜0.05）。结论：上前牙全瓷冠修复中，不同牙龈生物型对修复结果有不同影响，厚龈生物型修复后的美学效果 更佳，牙龈周边的颜色满意度更高，牙龈边缘退缩程度较小，但牙龈健康程度较差。
Abstract:
Objective To investigate infl uence of diff erent gingival biotypes in the anterior maxillary aesthetic zone on the tooth aesthetic eff ect and retraction in patients with all-ceramic anterior restorations. Methods Seventy-two patients received all ceramic anterior restorations in author’s hospital from April 2019 to February 2021 were enrolled, and classifi ed into two groups according to the probing depth measurement results, thin gingival group (n=29) and thick gingival group (n=43). One year after restoration, the aesthetic eff ect, gingival index, color satisfaction and dental gingival recession were compared between groups. Results The score of soft tissue morphology yielded no statistical diff erence between two groups (P ＞0.05), while the scores of the soft tissue color, texture and pink esthetic score (PES) of thick gingival group were signifi cantly higher than those of thin gingival group (P ＜0.05). An increasing trend was observed in gingival index in both groups, and the increase was more remarkable in thick gingival group than in thin gingival group (P＜0.05). The gingival margin color satisfaction (GMCS) score was decreased in thin gingival group, while was increased in thick gingival group, so the GMCS score of thick gingival group was signifi cantly higher than that of thin gingival group,the diff erences were statistically signifi cant (P ＜0.05). There was no gingival recession in either group before restoration, and the gingival recession was signifi cantly less in thick gingival group than in thin gingival group after restoration,the diff erences was statistically signifi cant(P ＜0.05). Conclusion In the restoration of full porcelain crown of upper anterior teeth, diff erent gingival biotypes have diff erent eff ects on the restoration results. The aesthetic eff ect of thick gingival biotype restoration is better, the color satisfaction around the gingiva is small, the degree of gingival margin recession is small, but the degree of gingival health is poor.
