摘要:
目的：观察品管圈（Quality control circle，QCC）活动联合心理护理对肥胖初产妇剖宫产术后整体切口愈合情况 及美观满意度的影响。方法：选择2021年5月-2022年1月笔者医院产科收治的110例肥胖初产妇为研究对象，采用随机数字表 法分为对照组（55例）和观察组（55例）。剖宫产术分娩术后对照组采用常规护理联合心理护理进行干预，观察组在对照组 基础上采用品管圈活动进行干预。观察两组术后恢复情况、干预完成后切口愈合情况、瘢痕形成情况、切口美观满意度及 住院期间并发症发生情况，比较两组产后1、3个月切口瘢痕相关评分。结果：观察组术后首次排气时间、首次下床活动时 间、首次排便时间、住院时间均短于对照组（P＜0.05）；产后1个月，观察组切口愈合优于对照组（P＜0.05）。产后3个 月，观察组瘢痕厚度、瘢痕长度、瘢痕宽度数值均低于对照组（P＜0.05），增生性瘢痕发生率低于对照组（P＜0.05）；产 后1、3个月，观察组温哥华瘢痕量表（Vancouver scar scale，VSS）、观测者瘢痕评价量表（Observer scar assessment scale，OSAS）、患者瘢痕评价量表（Puerpera scar assessment scale ，PSAS）评分均低于对照组（P＜0.05）；观察组并 发症发生率低于对照组（P＜0.05）；产后3个月，观察组切口美观满意率高于对照组（P＜0.05）。结论：采用QCC活动联合 心理护理对剖宫产术后肥胖初产妇进行干预，可有效提高产妇切口愈合效果，改善瘢痕形成情况，减少并发症的发生，因此 可有效提高产妇对切口愈合的美观满意度。
Abstract:
Objective To observe the eff ect of quality control circle activities and psychological nursing on the overall wound healing and aesthetic satisfaction of obese primiparas after Caesarean section. Methods A total of 110 obese primipara treated in the obstetrics department of author’s hospital from May 2021 to January 2022 were selected as the research objects. They were randomly divided into a control group (55 cases) and an observation group (55 cases) using a random number table method.After Caesarean section delivery, the control group was intervened by routine nursing combined with psychological nursing,and the observation group was intervened by quality control circle activities on the basis of the control group.Observe the postoperative recovery, incision healing after intervention, scar formation, satisfaction with incision aesthetics, and incidence of complications during hospitalization between the two groups, and compare the scores related to incision scars between the two groups at 1 and 3 months after surgery. Results The fi rst postoperative exhaust time, the fi rst time to get out of bed,the fi rst defecation time and the length of hospital stay in the observation group were shorter than those in the control group (P ＜0.05). One month after delivery,incision healing in the observation group was better than that in the control group (P ＜0.05). At 3 months postpartum, the values of scar thickness, scar length and scar width in the observation group were lower than those in the control group (P＜0.05), and the incidence of hypertrophic scar was lower than that in the control group (P＜0.05). At 1 and 3 months postpartum,vancouver scar scale (VSS) and observer scar assessment scale (OSAS) and puerpera scar assessment scale (PSAS) scores of observation group were lower than those of control group (P ＜0.05).The complication rate of observation group was lower than that of control group (P＜0.05). At 3 months postpartum,the aesthetic satisfaction rate of incision in observation group was higher than that in control group (P ＜0.05). Conclusion The intervention of QCC activity combined with psychological nursing on obese primiparas after cesarean section could eff ectively improve the incision healing eff ect of obese primiparas,improve scar formation,reduce the occurrence of complications,so it could eff ectively improve the satisfaction of obese primiparas with incision aesthetics.
