摘要:
目的：分析改良负压封闭引流技术联合集束化护理对断肢再植患者术后创面修复效果、疼痛应激及并发症的影响。 方法：选取2017年2月-2022年9月笔者医院收治的68例断肢再植术后患者为研究对象，将其中接受常规护理干预的32例患 者作为对照组，接受改良负压封闭引流技术联合集束化护理干预的36例患者作为研究组，比较两组患者的创面敷料维持 时间、创面愈合时间、致痛因子指标[P物质（Substance P，SP）、前列腺素E2（Prostaglandin E2，PGE2）、神经肽Y （Neuropeptide Y，NPY）]、疼痛视觉模拟评分（Visual analogue scale，VAS）、并发症及护理满意度。结果：术后， 研究组患者的创面敷料维持时间长于对照组，创面愈合时间短于对照组，并发症发生率低于对照组，差异均有统计学意义 （P＜0.05）；术后30 d，研究组患者的疼痛因子SP、PGE2、NPY水平及VAS评分均低于对照组（P＜0.05），护理满意度高于 对照组（P＜0.05）。结论：给予断肢再植术后患者改良负压封闭引流技术联合集束化护理措施，可有效提高患者创面修复 效果，加快创面愈合，降低疼痛应激程度，并减少并发症的发生，患者护理满意度也有所增加。
Abstract:
Objective To analyze the eff ects of improved vacuum sealing drainage (VSD) combined with cluster nursing on wound repair eff ect,pain stress and complications in patients after replantation of severed limb. Methods A total of 68 patients undergoing replantation of severed limb in the hospital were enrolled as the research objects between February 2017 and September 2022,including 32 cases given routine nursing in control group and 36 cases given improved VSD combined with cluster nursing in study group.The wound healing time,maintenance time of wound dressings,pain score [visual analogue scale (VAS)],pain indexes [substance P (SP),prostaglandin E2 (PGE2),neuropeptide Y (NPY)],the occurrence of complications and nursing satisfaction in the two groups were compared. Results After the operation,the wound dressing maintenance time of the study group was longer than that of the control group,the wound healing time was shorter than that of the control group,and the incidence of complications was lower than that of the control group,and the diff erences were statistically signifi cant (P ＜0.05). Thirty days after the surgery,the levels of pain factors SP,PGE2,NPY and VAS score in the study group were lower than those in the control group (P ＜0.05),and the nursing satisfaction was higher than that in the control group (P＜0.05). Conclusion Improved VSD combined with cluster nursing can accelerate wound healing,reduce changing times of dressings,pain stress and complications,and improve nursing satisfaction in patients after replantation of severed limb.
