摘要:

目的：探讨全程数字化导板辅助的即刻种植即刻修复在前牙美学区的精准性。方法：选择75例上前牙即刻种植的患者 作为研究对象，随机分为三组，每组25例。分别对其实施全程数字化导板下即刻种植即刻修复（A组）、常规即刻种植即刻 修复（B组）和常规即刻种植延期修复（C组），术后测量三组术前对其CBCT设计的虚拟理想位置与术后实际种植体植入的颈 部、根尖部的距离及其角度的偏差。通过在终修复体完成后当天、1个月、3个月、6个月软组织的红色美学评分来比较三组 的美学效果。结果：A组的颈部距离偏差、根尖距离偏差、角度偏差均小于B组和C组（P＜0.05），B组和C组差异无统计学意 义（P＞0.05）。A组修复完成后当天、1个月、3个月、6个月红色美学评分均高于B组和C组（P＜0.05）；B组修复完成后当 天、1个月、3个月、6个月红色美学评分均高于C组（P＜0.05）。结论：全程数字化导板辅助的即刻种植即刻修复在前牙美 学区可明显提高种植体植入位点的精准度和美学效果。 [

Abstract:

Objective To investigate the precision of immediate implant placement and immediate restoration in the aesthetic area of anterior teeth using fully guided digital guide. Methods Seventy-fi ve patients who needed immediate implant of maxillary anterior teeth were selected as the research subjects of this experiment. The patients were randomly divided into three groups, with 25 patients in each group. Those who were treated with fully guided digital immediate implant placement and immediate restoration (group A) and conventional immediate implant placement and immediate restoration (group B) and conventional immediate implant delayed restoration (group C) were compared in terms of the deviation between the virtual ideal position designed by CBCT before surgery and degree of the actual implant neck and apex after surgery. The aesthetic eff ects of three groups were evaluated using the pink esthetic scores of soft tissues at day 1,1 month, 3 months, and 6 months after the fi nal restoration of teeth. Results The implantation deviations( neck,apex,degree)of the group A were smaller than those of the group B and group C (P＜0.05). There was no signifi cant diff erence in deviation between Group B and Group C (P ＞0.05).The pink esthetic scores of the group A were signifi cantly higher than those of the group B and group C at day 1,1 month, 3 months, and 6 months after completion of repair(P ＜0.05), The pink esthetic scores of the group B were signifi cantly higher than group C at day 1,1 month, 3 months, and 6 months after completion of repair (P ＜0.05). Conclusion The precision and aesthetic eff ect of implantation in the aesthetic area of anterior teeth can be signifi cantly improved by using fully guided digital guide for immediate implant placement and immediate restoration.
