摘要:
目的：探究富血小板血浆（Platelet-rich plasma，PRP）联合自体皮片移植用于下肢慢性溃疡修复的近期和远期效果。 方法：选取2018年1月-2022年5月笔者医院收治的100例下肢慢性溃疡患者，根据治疗方式的不同分为观察组（n=52）和对照组 （n=48）。对照组给予人工真皮联合自体皮片移植治疗；观察组给予PRP联合自体皮片移植治疗。比较两组创面修复相关指标、 术后疼痛、临床疗效及瘢痕情况。结果：观察组总有效率高于对照组，肉芽长出及创面愈合时间均短于对照组（P＜0.05）。植 皮术后1 d，观察组疼痛评分明显低于对照组（P＜0.05）；植皮术后5 d，两组疼痛评分比较，差异无统计学意义（P＞0.05）。两 组植皮术后10 d皮片存活率比较，差异无统计学意义（P＞0.05）。观察组术后1、3、6、12个月的瘢痕评分显著低于对照组 （P＜0.05）；重复方差分析显示，两组存在组间、时间及交互效应（均P＜0.001）。结论：PRP联合皮片移植用于下肢慢性溃 疡修复，有助于提高临床疗效，缩短肉芽长出时间和创面愈合时间，减轻患者术后疼痛感受，并能减少术后瘢痕形成。
Abstract:
Objective To analyze the short-term and long-term eff ects of platelet rich plasma (PRP) combined with skin graft transplantation for the repair of chronic lower limb ulcers. Methods A total of 100 patients with chronic lower limb ulcer admitted to the author’s hospital from January 2018 to May 2022 were selected and divided into observation group (n=52 cases) and control group (n=48 cases) according to diff erent treatment methods. The control group was treated with artifi cial dermis combined with autologous skin grafting. The observation group was treated with PRP combined with autologous skin grafting. The wound repair related indicators, postoperative pain, clinical effi cacy, and scar condition were compared between the two groups. Results The total eff ective rate of the observation group was higher than that of the control group, and the granulation growth and wound healing time were shorter than those of the control group (P＜0.05). At 1 day after skin grafting, the pain score of the observation group was signifi cantly lower than that of the control group (P＜0.05). There was no signifi cant diff erence in the pain score between the two groups 5 days after skin grafting (P＞ 0.05). There was no signifi cant diff erence in the survival rate of skin grafts between the two groups 10 days after skin grafting (P＞0.05). The scar scores of the observation group at 1, 3, 6 and 12 months after operation were signifi cantly lower than those of the control group (P＜0.05). Repeated analysis of variance showed that there were inter-group, time and interaction eff ects between the two groups (all P＜0.001). Conclusion PRP skin graft is used to repair chronic ulcer of lower limbs, which is helpful to improve the clinical effi cacy, shorten the time of granulation and wound healing, reduce the postoperative pain of patients and reduce the risk of scar formation.
