摘要:
目的：探讨即刻种植对Ⅲ、Ⅳ类骨质前牙单牙缺失患者修复效果及美学指数的影响。方法：选取2017年8月-2019年8 月在笔者医院接受上颌前牙单牙种植的124例牙缺失患者，按照随机数字表法分为即刻种植组62例（即刻种植处理），其中 Ⅲ、Ⅳ类骨质分别为49例和13例；对照组62例（常规种植处理），其中Ⅲ、Ⅳ类骨质分别为51例和11例。分别采用红色美学 指数（Pink ethetic score,PES）、牙龈乳头指数（Gingival nipple index,GNI）、种植体稳定性（Implant stability quality,ISQ）评价患者手术前后美学指数及稳定性。结果：基线期，即刻种植组较对照组的边缘龈水平、近中龈乳头等PES 指数评分高，差异有统计学意义（P＜0.05）；治疗1年后，两组PES指数评分相近（P＞0.05），即刻种植组与对照组 各级GNI指数差异均无统计学意义（P＞0.05）。即刻种植组的Ⅲ、Ⅳ类骨质治疗时间较对照组短，差异有统计学意义 （P＜0.05）。即刻种植和常规种植的ISQ值、并发症及成功率比较差异无统计学意义（P＞0.05）。结论：即刻种植治疗 Ⅲ、Ⅳ类骨质前牙单牙缺失能明显缩短治疗周期，提升基线期PES评分，具有良好的修复及美学效果,有较高的临床应用价值。
Abstract:
Objective To investigate the effect of immediate implantation on the restoration effect and aesthetic index of patients with Class Ⅲ and Ⅳ bone loss of anterior teeth. Methods A total of 124 patients with single tooth loss from August 2017 to August 2019 in our hospital who was prepared to accept single maxillary anterior teeth tooth planting were selected and divided into the immediate implantation group of 62 patients (immediate implant treatment, including Ⅲ 49 and Ⅳ 13 cases respectively) and control group of 62 cases (conventionally grown processing, including Ⅲ 53 and Ⅳ 11 cases respectively) according to random number table method. Red aesthetic index (PES), gingival nipple index (GNI) and implant stability (ISQ) were used to evaluate the aesthetic index and stability of patients before and after surgery. Results At baseline, the immediate implantation group had higher PES index scores (marginal gingival level and mesial gingival papilla) than the control group (P＜0.05). After 1 year of treatment, the PES index scores of the two groups were similar (P＞0.05). There was no significant difference in GNI index between the immediate implantation group and the control group (P＞0.05). The treatment time for bone type Ⅲ and IV in the immediate implantation group was shorter than that in the control group (P ＜0.05). There was no significant difference in ISQ value, complications and success rate between two groups (P＞0.05). Conclusion Immediate implantation treatment for patients with boneless anterior teeth of type Ⅲ and IV can significantly shorten the treatment period, improve the baseline PES score, and have good restoration and aesthetic effects, and high clinical application value
