摘要:
目的：分析度普利尤单抗辅助治疗中重度特应性皮炎（Atopic dermatitis,AD）的疗效及对外周血Th1/Th2细胞的调 控作用。方法：选择笔者科室2019年1月-2020年6月诊治的134例AD患者，采用计算机随机数字分组法分成对照组和治疗组， 每组67例。对照组患者接受基础治疗，治疗组患者使用度普利尤单抗辅助治疗。治疗结束后评定两组患者外周血Th1/Th2 比值，血清总IgE、血嗜酸性粒细胞(EOS)计数和皮损改善情况，以及湿疹面积和严重程度、临床疗效、疼痛及瘙痒情况 和不良反应发生情况。结果：治疗前，两组患者外周血Th1/Th2比值，血清总IgE及EOS计数差异比较，差异均无统计学意 义（P＞0.05）；治疗后，治疗组患者外周血Th1/Th2比值高于对照组（P＜0.05），治疗组血清总IgE及EOS计数均低于对照 组，差异均有统计学意义（P＜0.05）。治疗前，两组皮损改善评分、EASI评分比较，差异均无统计学意义（P＞0.05）；治 疗后，治疗组患者皮损改善评分、EASI评分均低于对照组，差异均有统计学意义（P＜0.05），治疗组患者临床总有效率高 于对照组（P＜0.05）。治疗前两组患者疼痛及瘙痒评分差异比较无统计学意义（P＞0.05），治疗后，治疗组患者疼痛及瘙 痒评分均低于对照组（P＜0.05），且治疗组患者不良反应发生率低于对照组，差异均有统计学意义（P＜0.05）。结论：度 普利尤单抗辅助治疗对中重度AD能提升患者临床疗效及免疫功能，建议临床推广应用。
Abstract:
Objective To analyze the aesthetic effect of dupilumab in the treatment of moderate-to-severe atopic dermatitis (AD) and its regulation in peripheral blood T helper 1 (Th1)/T helper 2 (Th2) cells. Methods With the approval of hospital ethics committee, a sample of 134 AD patients diagnosed in hospital from January 2019 to June 2020 were selected, and randomly divided into control group and treatment group using computer, each with 67 cases. Control group received routine treatment, while treatment group received dupilumab treatment. After treatment, the peripheral blood Th1/Th2 ratio, serum total immunoglobulin E (IgE), eosinophil count (EOS), improvement of skin lesions, eczema area severity index (EASI), clinical eff ect, pain degree, itching degree and adverse reactions were compared between the two groups. Results The peripheral blood Th1/Th2 ratio, serum total IgE and EOS count yielded no statistical difference between two groups before treatment (P＞ 0.05). After treatment, Th1/Th2 ratio in treatment group, which was signifi cantly higher than that of control group (P＜0.05), meantime, serum IgE and EOS count of treatment group were signifi cantly lower than those of control group (P＜0.05). The improvement of skin lesions and EASI score demonstrated no signifi cant diff erence between two groups (P＞0.05), and the improvement of skin lesions and EASI score in treatment group after treatment were signifi cantly lower than those of control group (P＜0.05). The overall effi cacy rate was signifi cantly higher in treatment group than in control group (P＜0.05). The scores of pain and itching denoted no signifi cant diff erence between two groups before treatment (P＞0.05). After treatment, the scores of treatment group, which were signifi cantly lower than those of control group (P＜0.05). The overall adverse reaction rate was lower in treatment group than in control group (P＜0.05). Conclusion Application of dupilumab inthe treatment of moderate-to-severe AD achieves good aesthetic eff ect and eff ectively improves the immune function.
