目的:评估浓缩生长因子纤维蛋白(Concentrated growth factors fibrin,CGF)在美学区即刻种植术后软组织愈 合中的作用。方法:选择美学区即刻种植患者70例,分为实验组36例和对照组34例。实验组种植周围骨缺损间隙内植入 CGF+Bio-Oss混合物,牙槽嵴顶分层覆盖Bio-Guide生物膜及CGF膜片,对照组种植周围骨缺损间隙内植入Bio-Oss骨颗粒,牙 槽嵴顶覆盖Bio-Guide生物膜。观察术前及术后6个月的唇侧骨板情况,术后第1、3、7天种植区软组织炎症状态及患者的肿 痛不适程度,术前与术后第3、6个月的软组织愈合状态。结果:实验组患者具有更低的种植后术区疼痛评价(VAS评分), 更良好的种植术后术区炎症状态,即较轻的术区肿胀、较少的术区出血以及更健康的术区黏膜颜色;并且相较于对照组,实 验组患者的角化龈宽度更宽,其种植术后红色美学指数更高;以上指标组间比较差异均有统计学意义(P<0.05)。而对于 两组患者来说,即刻种植体种植成功率、唇侧骨量及角化龈厚度比较差异并无统计学意义(P>0.05)。结论:浓缩生长因 子纤维蛋白能有效提高美学区即刻种植后软组织的愈合速度及效果,降低局部炎症反应,减轻术区疼痛不适感。
Abstract:
Objective To evaluate the role of concentrated growth factor fi brin (CGF) in soft tissue healing after immediate implantation in the aesthetic zone. Methods Seventy patients with immediate implantation in the aesthetic zone were selected and divided into an experimental group of 36 patients and a control group of 34 patients. In the experimental group, CGF+BioOss mixture was implanted in the peri-implant bone defect gap, and the top of the alveolar ridge was covered with Bio-Guide and CGF biofi lm in layers, while in the control group, Bio-Oss bone particles were implanted in the peri-implant bone defect gap, and the top of the alveolar ridge was covered with Bio-Guide biofi lm. The soft tissue healing status of the implant area and the degree of swelling, pain and discomfort of the patients at diff erent time point were evaluated after the post-implantation. Results The patients in the experimental group had lower post-implantation pain assessment (VAS score), better post implantation infl ammation status, i.e., less swelling, less bleeding, and healthier mucosal color, and wider keratinized gingival width and higher post-implantation red aesthetic index compared to the control group, the diff erences between the two groups were statistically signifi cant (P <0.05). The diff erence in immediate implant placement success rate, labial bone volume and keratinized gingival thickness was not statistically signifi cant between the two groups (P >0.05). Conclusion Concentrated growth factor fibrin can effectively improve the healing speed and effect of soft tissue after immediate implantation in the aesthetic area, reduce the local infl ammatory reaction, and alleviate the pain and discomfort in the operative area.
