Effi cacy of Compound Shouwu Jili Decoction Combined with Fire Needle and Tacrolimus in the Treatment of Stable Vitiligo

摘要:

目的：探讨复方首乌蒺藜汤联合火针及他克莫司治疗稳定期白癜风的疗效。方法：选取笔者医院2019年1月-2021年1 月收治的125例稳定期白癜风患者，按照随机数字表法分为对照组63例（火针治疗）和观察组62例（火针+复方首乌蒺藜汤 治疗）。此外，两组均给予局部外用0.1%他克莫司软膏涂抹治疗。治疗6个月后，比较两组临床疗效，治疗前后两组间中医 证候积分，皮损色素恢复效果，T淋巴细胞亚群、氧化应激指标水平，记录不良反应发生情况。结果：治疗后，观察组总有 效率（91.94%）显著高于对照组（79.37%）（P＜0.05）；治疗后，两组各项中医证候积分均下降（P＜0.05），且观察组 治疗后各项中医证候积分均低于对照组（P＜0.05）；治疗后，两组靶皮损白斑面积均显著下降（P＜0.05），皮损色素积 分显著上升（P＜0.05），且观察组治疗后靶皮损白斑面积小于对照组，皮损色素积分高于对照组（P＜0.05）；治疗后， 两组CD3+ 、CD4+ 、CD4+ /CD8+ 均显著上升（P＜0.05），CD8+ 显著下降（P＜0.05），且观察组改善优于对照组（P＜0.05）；治 疗后，两组超氧化物歧化酶（Superoxidedismutase，SOD）、血浆总抗氧化能力（Total antioxidant capacity，T-AOC） 和谷胱甘肽过氧化物酶（Glutathione peroxidase，GSH-Px）水平均显著上升（P＜0.05），丙二醛（Malondialdehyde， MDA ）显著下降（P＜0.05），且观察组SOD、T-AOC及GSH-Px水平高于对照组，MDA水平低于对照组（P＜0.05）；两组患者不 良反应发生情况比较无统计学意义（P＞0.05）。结论：复方首乌蒺藜汤联合火针疗法治疗稳定期白癜风患者，能有效提高 临床疗效，促进皮损色素恢复，改善患者免疫状态和氧化应激失衡。

Abstract:

Objective To study the effi cacy of compound Shouwu Jili decoction combined with fi re needle in treating stable vitiligo and its infl uence on the recovery eff ect of skin pigment. Methods 125 patients with stable vitiligo admitted to the hospital from January 2019 to January 2021 were selected and divided into control group (63 cases, fi re needle therapy) and observation group (62 cases, fi re needle+compound Shouwu Jili decoction) according to the random number table method. All patients were given topical application of 0.1% tacrolimus ointment. After 6 months of treatment, the clinical efficacy, and scores of TCM syndromes, skin pigment recovery eff ect, T lymphocyte subsets and oxidative stress indicators before and after treatment were compared between the two groups, and the occurrence of adverse reactions were recorded. Results After treatment, the total eff ective rate in observation group (91.94%) was signifi cantly higher than that in control group (79.37%) (P ＜0.05). After treatment, the scores of TCM symptoms in the two groups were decreased (P＜0.05), and the scores of TCM symptoms in observation group after treatment were lower than those in control group (P＜0.05). After treatment, the white spot area of target skin lesions in the two groups was signifi cantly reduced and the skin pigment score was signifi cantly enhanced (P ＜0.05). And the observation group improved better than the control group (P＜0.05). After treatment, CD3+ , CD4+ and CD4+ /CD8+ in both groups were signifi cantly increased (P ＜0.05) while CD8+ was signifi cantly decreased (P ＜0.05), and the observation group improved better than the control group(P＜0.05). After treatment, the levels of SOD, T-AOC and GSHPx in the two groups were signifi cantly risen (P ＜0.05) while the level of MDA was signifi cantly declined (P ＜0.05), and the indicators in observation group were signifi cantly diff erent from those in control group after treatment (P ＜0.05). There were no statistical diff erences in the occurrence of adverse reactions between the two groups (P ＞0.05). Conclusion Compound Shouwu Jili decoction combined with fi re needle therapy in the treatment of patients with stable vitiligo can eff ectively enhance the clinical effi cacy, promote the recovery of skin pigment, improve the immune status and oxidative stress imbalance.
