摘要:
目的:探究Nd∶YAG激光辅助治疗牙周炎对患者骨代谢、牙周相关指标及炎性因子水平的影响。方法:选取2017年1 月-2020年4月笔者医院收治的60例牙周炎患者,采用随机数字表法分为对照组和观察组,各30例。对照组采用超声龈下刮治 术和根面平整术进行治疗,观察组在对照组基础上采用Nd∶YAG激光辅助治疗,治疗后3个月观察治疗效果。比较两组治疗后 3 个月总有效率,治疗前、治疗后3个月牙周指标、龈沟液骨钙素(Osteocalcin,OCN)、Ⅰ型胶原(Type Ⅰ collagen , COL- Ⅰ)、核因子κB受体活化因子配体(Nuclear factor κB receptor activating factor ligand ,RANKL)、炎性因 子水平。结果:治疗后3个月,观察组总有效率为96.67%,高于对照组的76.67%(P<0.05)。治疗后3个月,两组牙菌斑 指数(Plaque index,PLI)及牙龈指数(Gingival index,GI)评分、牙周袋深度(Pocket depth,PD)水平,龈沟液 RANKL 、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白细胞介素6(Interleukin 6,IL-6)、白细胞介素 1β (Interleukin 1β,IL-1β)水平较治疗前均降低,且观察组低于对照组(P<0.05);而两组龈沟液OCN、COL-I水平 较治疗前升高,观察组高于对照组(P<0.05)。结论:Nd∶YAG激光辅助治疗牙周炎可提高患者龈沟液OCN、COL-I水平,降 低龈沟液RANKL水平,改善骨代谢,抑制炎症反应,进而改善牙周相关指标,疗效确切。
Abstract:
Objective To explore the effects of Nd∶YAG laser assisted in treating periodontitis on bone metabolism and periodontal esthetics of patients. Methods Sixty patients with periodontitis admitted to the author’s hospital from January 2017 to April 2020 were selected and divided into control group and observation group by random number table method, with 30 patients in each group. The control group was treated with ultrasonic subgingival scaling and root surface leveling, and the observation group was treated with Nd∶YAG laser on the basis of the control group, the therapeutic eff ect was observed 3 months after treatment. The total effective rate of the two groups at 3 months after treatment was compared, levels of periodontal indexs, gingival crevicular fl uid osteocalcin (OCN), type i collagen (COL-I), nuclear factor κB receptor activating factor ligand (RANKL) and inflammatory factors before treatment and at 3 months after treatment of the two groups were compared. Results At 3 months after treatment,the total eff ective rate of the observation group was 96.67%, higher than that of the control group (76.67%)(P <0.05). At 3 months after treatment, plaque Index (PLI), gingival Index (GI) scores, pocket depth (PD) level, RANKL 、tumor necrosis factor-α (TNF-α), interleukin 6 (IL-6), interleukin 1β (IL-1β) levels of gingival crevicular fl uid in both groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P <0.05). At 3 months after treatment, OCN, CoL-I levels of gingival crevicular fl uid in the two groups were higher than those before treatment, and those in the observation group were higher than those in the control group (P <0.05). Conclusion Nd∶YAG laser assisted in treating periodontitis could improve OCN and COL-I levels of gingival crevicular fl uid, reduce RANKL level of gingival crevicular fl uid, improve bone metabolism, inhibit infl ammatory response, and then improve periodontal aesthetics esthetics indexs, with eff ective eff ect.
