摘要:
目的：初步探究无托槽隐形矫治器对替牙期上颌轻中度狭窄患者的扩弓效果。方法：选取2019年1月-2021年12月于 笔者医院口腔正畸科就诊的处于替牙期的上颌轻中度狭窄的患者15例，并使用无托槽隐形矫治器完成上颌扩弓。通过测量 三维数字化牙列模型，测量矫治前后各牙位的牙弓宽度，计算各牙位的扩弓量及扩弓效率。结果：与矫治前相比，矫治后 各牙位的牙弓宽度均有增加，差异有统计学意义（P＜0.05），但各牙位实际扩弓量均小于预设扩弓量，差异有统计学意义 （P＜0.05）。乳尖牙、第一乳磨牙、第二乳磨牙、第一恒磨牙的扩弓效率分别为（68.3±6.58）%、（74.8±7.83）%、 （72.5±8.37）%、（71.2±7.49）%。结论：在上颌骨轻中度狭窄情况下，无托槽隐形矫治器能够在一定程度上增加替牙列 期间上颌牙弓的宽度，有较好的扩弓效果，临床在把握适应证的前提下可以多加应用。
Abstract:
Objective To investigate the effect of clear aligners on maxillary expansion in patients with mild to moderate maxillary stenosis in mixed dentition. Methods Fifteen patients with mild to moderate maxillary stenosis in mixed dentition were selected from the Department of Orthodontics of the author’s hospital from January 2019 to December 2021, and maxillary expansion was completed with clear aligners. The arch width of each tooth before and after treatment was measured by the three-dimensional digital dentition model, and th expansion volume and expansion efficiency of each tooth were calculated. Results After treatment, the arch width of each tooth was increased, and the difference was statistically significant (P ＜0.05). However, the actual expansion of each tooth was less than the preset expansion, and the diff erence was statistically signifi cant (P ＜0.05). The expansion effi ciency of primary canine, fi rst primary molar, second primary molar, and fi rst permanent molar was (68.3±6.58) %, (74.8±7.83) %, (72.5±8.37) %, and (71.2±7.49) %, respectively. Conclusion In the case of mild to moderate maxillary stenosis, the clear aligners can increase the width of the maxillary arch during the mixed dentition to some extent, and has a good eff ect on arch expansion, which can be used more under the clinical indications.
