摘要:
目的：分析正畸固定牵引断根结合修复治疗在前牙龈下冠折美学修复中的临床应用效果。方法：选取2020年5月-2023 年5月笔者医院收治的76例前牙龈下冠折患者，随机分为对照组（n=38）和观察组（n=38）。两组患者均给予牙髓摘除根管 治疗，根管治疗1个月后，对照组切除牙龈后行烤瓷冠修复，观察组给予正畸固定牵引断根结合烤瓷冠修复。随访1年，记录 两组患者治疗优良率、前牙咬合功能、牙周健康情况、美学效果、总治疗时长、治疗花费、满意度及并发症发生情况，并进 行统计学分析。结果：观察组治疗优良率为97.37%，明显高于对照组的84.21%（P＜0.05）；观察组前牙咬合功能恢复情况 优于对照组，牙尖交错位肌电值及最大紧咬合状态肌电值明显高于对照组（P＜0.05）；观察组牙周健康情况优于对照组， 红色美学指数及白色美学指数明显高于对照组（P＜0.05）；观察组总治疗时长与治疗花费均明显高于对照组（P＜0.05）； 观察组患者满意度为97.37%，明显高于对照组的73.68%，且观察组各项并发症发生率均明显低于对照组（P＜0.05）。结 论：正畸固定牵引断根结合前牙美学修复治疗前牙龈下冠折疗效肯定，有助于前牙咬合功能恢复，维持牙周健康及美学效 果，降低并发症发生率，提高患者满意度。
Abstract:
Objective To study the application eff ect of orthodontic fi xed traction combined with root fracture repair treatment in aesthetic restoration of anterior teeth. Methods 76 patients with anterior subgingival crown fold in front teeth admitted to the author’s hospital from May 2020 to May 2023 were randomly divided into a control group (n=38) and an observation group (n=38). Both groups of patients were treated with extirpation of pulp and root canal therapy. After one month of root canal therapy, the control group underwent gingival resection and porcelain restoration, while the observation group received orthodontic fixed traction combined with porcelain restoration treatment. Follow up for 1 year , the excellent and good treatment rates, anterior occlusal function, periodontal health, aesthetic effects, total treatment duration, treatment cost, satisfaction, and incidence of complications of two groups of patients were compared. Results The excellent and good treatment rate in observation group was significantly higher than the control group (97.37% vs. 84.21%, P ＜0.05).The restoration of anterior teeth occlusion function in observation group was higher than the control group, and the IPC-HB and IPC-MC electromyographic values were signifi cantly higher(P ＜0.05).The periodontal health status of the observation group was better than the control group, with higher PES and WES indexes(P＜0.05). The total treatment duration and treatment cost of the observation group were signifi cantly higher than those of the control group (P ＜0.05). The satisfaction rate of patients in the observation group was higher than the control group(97.37% vs. 73.68%), and the incidence of various complications was signifi cantly lower(P ＜0.05). Conclusion The orthodontic fi xed traction combined with root fracture repair treatment of aesthetic defects in front teeth had a positive eff ect, can help to restore the occlusal function of the anterior teeth, maintain periodontal health and aesthetic eff ects, reduce the incidence of complications, and improve patient satisfaction.
