摘要:
目的:探究根管治疗(Root Canal Treatment,RCT)联合不同树脂治疗Ⅱ类洞牙体缺损合并慢性根尖周炎(Chronic Apical Periodontitis,CAP)的临床效果。方法:选取2022年1月-2023年1月笔者医院收治的104例(共186颗患牙)Ⅱ类洞牙体缺损合并CAP的患者作为研究对象,采用电脑随机数字抽取法分为 Z350组(RCT联合3M FiltekTM Z350纳米复合树脂治疗,n=52,患牙95颗)和SonicFill组(RCT联合SonicFill超声树脂治疗,n=52,患牙91颗)。比较两组治疗效果,治疗前与治疗后3个月的疼痛情况、牙周指标[牙龈指数(Gingival Index,GI)、菌斑指数(Plaque Index,PI)]及龈沟液炎症因子[肿瘤坏死因子α(Tumor Necrosis Factor Receptor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)],随访1年牙体修复情况。结果:Z350组、SonicFill组治疗总有效率(94.23% vs.98.08%)比较,差异无统计学意义(P>0.05);治疗后3个月,SonicFill组疼痛评分低于Z350组(P<0.05);两组治疗前后GI、PI比较,差异均无统计学意义(P>0.05);治疗后3个月,与Z350组比较,SonicFill组龈沟液炎症介质TNF-α、IL-6水平更低(P<0.05);两组随访1年的牙体表面形态、边缘着色、边缘完整性、继发龋、食物嵌塞、充填敏感等牙体修复情况比较,差异无统计学意义(P>0.05)。结论:RCT联合SonicFill树脂治疗Ⅱ类洞牙体缺损合并CAP与RCT联合3M FiltekTM Z350纳米复合树脂治疗效果相近,均能够较好地修复牙体缺损,RCT联合SonicFill超声树脂治疗可更好地缓解患者疼痛感与龈沟液炎症。
