摘要:
目的:探讨局部氧疗(Topical oxygen therapy,TOT)与负压封闭引流(Vacuum sealing drainage,VSD)联合治疗皮肤深Ⅱ度损伤的临床效果。方法:选取2018年10月-2020年10月在笔者医院治疗的159例皮肤深Ⅱ度损伤患者作为观察对象,按照随机数字表法将其分为观察组、对照组与传统组,每组各53例。传统组给予传统清创换药术,对照组给予VSD+生理盐水冲洗,观察组给予VSD+TOT治疗。治疗7d后比较三组创面愈合情况(创面肉芽组织覆盖率、创面组织液分压、创面愈合率、创面完全愈合时间)、创面细菌清除率、泡沫干瘪率、堵管率、住院时间,比较治疗前、治疗7d后三组患者血清炎性因子[白介素-6(Interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)]、致痛因子[5-羟色胺(5-hydroxytryptamine,5-HT)、脑内神经肽Y(Neuropeptide Y,NPY)、前列腺素E2(Prostaglandin E2,PGE2)]水平变化情况。结果:观察组与对照组创面组织覆盖率、创面愈合率、创面细菌清除率均高于传统组,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组与对照组创面完全愈合时间、住院时间均低于传统组,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组与对照组泡沫干瘪率、堵管率比较差异无统计学意义(P>0.05)。治疗前,三组血清炎性因子(IL-6、CRP、TNF-α)、致痛因子(5-HT、NPY、PGE2)比较差异无统计学意义(P>0.05);治疗7d后,观察组与对照组IL-6、CRP、TNF-α、5-HT、NPY、PGE2水平均低于传统组,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:TOT与VSD联合治疗皮肤深Ⅱ度损伤可有效清除创面细菌,降低炎性因子及致痛因子水平,促进创面愈合,缩短住院时间。
Abstract:
Objective To observe the effect of combined treatment of topical oxygen therapy (TOT) and vacuum sealing drainage (VSD) on wound healing in patients with deep second-degree skin injury. Methods Totally 159 patients with deep second-degree skin injury treated in the hospital between October 2018 and October 2020 were selected as observation subjects, and they were divided into the observation group, the control group and the traditional group according to the random number table method, with 53 cases in each group. Patients in the traditional group received traditional debridement and dressing change, and patients in the control group were treated with VSD+physiological saline flush, and patients in the observation group were treated with VSD+TOT. After 7 d of treatment, the wound healing status (coverage rate of wound granulation tissue, partial pressure of wound tissue fluid, wound healing rate, complete wound healing time), clearance rate of wound bacteria, foam shrinkage rate, tube blocking rate and hospital stay were compared among the three groups of patients. The changes in levels of serum inflammatory factors [Interleukin-6 (IL-6), C-reactive protein (CRP), Tumor necrosis factor- α (TNF- α)] and pain-causing factors [5-hydroxytryptamine (5-HT), Neuropeptide Y (NPY), Prostaglandin E2 (PGE 2)] of the three groups were compared before treatment and after 7 d of treatment. Results The wound tissue coverage, wound healing rate and wound bacterial clearance rate of the observation group and the control group were higher than those of the traditional group, and the observation group was higher than that of the control group, the differences were statistically significant (P <0.05). The time of complete wound healing and hospital stay in the observation group and the control group were lower than those in the traditional group, and the observation group was lower than that in the control group (P<0.05). There was no significant difference between the observation group and the control group in foam shrinkage rate and tube plugging rate (P>0.05). Before treatment, there were no significant difference in serum inflammatory factors (IL-6, CRP, TNF-α) and pain causing factors (5-HT, NPY, PGE2) among the three groups (P>0.05). After 7 days of treatment, the expression of IL-6, CRP, TNF-α, 5-HT, NPY, PGE2 in the observation group and the control group were lower than those in the traditional group, and in the observation group was lower than those in the control group (P<0.05). Conclusion TOT combined with VSD for patients with deep second-degree skin injury can effectively remove wound bacteria, reduce the levels of inflammatory factors and pain-causing factors, promote wound healing, and shorten the hospital stay
