摘要:
目的:探讨点阵激光治疗凹陷性痤疮瘢痕的疗效及安全性。方法:选取2015年7月-2018年5月笔者医院诊治的121例凹陷性痤疮瘢痕患者为研究对象,采用随机数字表法将患者分为对照组(n=60)和实验组(n=61),两组分别采用2940nm和1550nm铒玻璃点阵激光治疗,1次/4周,连续治疗3次。分别于治疗前后检测两组患者皮肤平整度变化、经皮水流失、皮肤含水量、黑色素等,统计治疗期间不良反应。结果:与治疗前比较,治疗3次后两组毛孔均缩小(P<0.05),紫色斑和棕色斑逐渐减少(P<0.05),且实验组第2次及第3次治疗后毛孔较对照组缩小更为显著(P<0.05)。与治疗前比较,治疗后1周两组经皮失水量增加、皮肤含水量减少及面部油脂增加(P<0.05),且两组间差异有统计学意义(P<0.05);术后1个月,两组上述数据又均恢复至治疗前水平(P>0.05)。治疗期间,实验组未出现渗血情况,与治疗后即刻比较,治疗后3d两组红斑、水肿、疼痛及对照组渗血程度均明显减轻(P<0.05),治疗后1个月两组上述不良反应均消失;与对照组比较,治疗后即刻实验组水肿及疼痛评分更高,治疗后3d水肿评分仍高于对照组,疼痛评分低于对照组,红斑数量少于对照组,差异有统计学意义(P<0.05)。结论:1550nm铒玻璃点阵激光较2940nm治疗凹陷性痤疮瘢痕效果更佳,且对患者皮肤屏障损伤更小。
Abstract:
Objective To investigate the efficacy and safety of fractional laser in the treatment of depressed acne scars. Methods 121 patients with depressed acne scar from July 2015 to May 2018 were randomly divided into control group (n=60) and experimental group (n=61), which were treated with erbium glass lattice laser at 2 940 nm and 1 550 nm respectively, 1 ti mes/4 weeks, all had continuous treatment for 3 times. The changes of skin smoothness, percutaneous water loss, skin water content and melanin were detected before and after treatment, and the side effects were counted. Results The pores of the 2 groups were gradually reduced and the purple, brown spots gradually decreased from before treatment to the third treatment (P<0.05), and the pores of the experimental group were significantly smaller than those of the control group after the second and third treatment (P<0.05). Compared with bofore each treatment, the skin water loss increased, skin water content decreased and facial oil increased in the 2 groups one week after each treatment (P<0.05), and the difference between the 2 groups was significant (P<0.05), the above data of the 2 groups restored to the pre-treatment levels one month after treatment(P>0.05). During the treatment, there was no bleeding occurred in the experimental group, and compared with immediately after treatment, the erythema, edema, pain of the 2 groups and bleeding of the control group significantly reduced 3 days after treatment(P<0.05), and the side effects of the 2 groups disappeared 1 month after treatment. Compared with the control group, the edema and pain scores of the experimental group increased immediately after treatment, but the edema score was still higher than that of the control group, and the pain score was lower than that of the control group 3 days after treatment, the number of erythema was less than that of the control group(P<0.05). Conclusion 1 550 nm erbium glass lattice laser is more effective than 2 940 nm in treating depressed acne scar, and the damage of skin barrier is less.
