摘要:
目的:分析微种植体支抗联合MBT矫治技术在青少年骨性Ⅱ类1分类患者中的应用价值。方法:以2017年9月-2019年7月 笔者医院就诊的64例青少年骨性Ⅱ类1分类患者为研究对象,按随机数字表法随机分为观察组(n=32)和常规组(n=32)。常 规组行MBT直丝弓联合传统支抗进行正畸治疗,观察组行微种植体支抗联合MBT矫治技术进行正畸治疗,观察两组的治疗效果, 比较治疗前后两组的解剖情况、咀嚼功能及侧貌软组织变化情况,并统计术后并发症发生情况。结果:治疗后,观察组总有效率 (90.63%)显著高于常规组(71.88%),差异有统计学意义(P<0.05);观察组上颌第一磨牙近中移动量小于常规组,差异有统 计学意义(P<0.05);两组患者上中切牙凸距和上中切牙倾斜角度较治疗前显著降低,观察组改变高于常规组(P<0.05);治 疗后,两组患者咬合力和咀嚼效能较术前显著提高,且观察组显著高于常规组,差异均有统计学意义(P<0.05);两组治疗后 的软组织面角较治疗前明显增大,颏前点距离明显减小,两组比较差异有统计学意义(P<0.05),两组治疗后的颏唇沟深 较治疗前均明显减小,但组间比较差异无统计学意义(P>0.05);术后24个月矫治期间,观察组并发症发生率(6.26%)低 于常规组(15.63%),但差异无统计学意义(P>0.05)。结论:微种植体支抗联合MBT矫治技术治疗青少年骨性Ⅱ类1分类 患者,能有效改善上颌前突、下颌后缩,提高患者面部美观度,增强其咀嚼能力,且安全性良好,值得临床推广应用。
Abstract:
Objective To analyze the application value of screw implant anchorage combined with MBT correction technique on adolescent skeletal class II division 1 cases. Methods 64 adolescent patients with skeletal class II division 1 malocclusion who were treated in the hospital between September 2017 and July 2019 were randomly divided into observation group (n=32) and routine group (n=32) according to the random number table method. Patients in the routine group were given MBT straight wire arch combined with traditional arch anchorage for orthodontic treatment, and patients in the observation group were treated with screw anchorage combined with MBT correction technique for orthodontic treatment. The eff ect after treatment of the two groups was observed, and the anatomy, chewing function and soft tissue profi le changes of the two groups were compared before and after treatment, and the occurrence of postoperative complications were statistically analyzed. Results After treatment, the total eff ective rate of observation group was signifi cantly higher than that of routine group (90.63% vs 71.88%) (P<0.05). After treatment, the mesial movement of maxillary fi rst molar of observation group was shorter compared with that of routine group (P<0.05). The upper central incisor protruding distance and upper central incisor inclination angle of the two groups were signifi cantly reduced compared with those before treatment, and the changes of observation group were greater than those of routine group (P<0.05). After treatment, the occlusal force and chewing effi ciency of the two groups of patients were signifi cantly higher than those before surgery (P<0.05), and the indicators were signifi cantly stronger in observation group than those in routine group (P<0.05). After treatment, the soft tissue surface angle was signifi cantly enlarged and the pogonion distance was significantly shortened compared to before treatment, and there were statistical differences between observation group and routine group (P<0.05), and the mentolabial sulcus depth was significantly decreased in the two groups, but there was no significant difference between the groups (P>0.05). During 24 months of correction after treatment, the incidence rate of complications with 6.26% in observation group was lower than 15.63% in routine group (P>0.05). Conclusion Screw implant anchorage combined with MBT correction technique can eff ectively improve the maxillary protrusion and mandible retraction, increase the facial appearance of patients and enhance the chewing ability, and ithas good safety and is worthy of clinical promotion.
