摘要:
目的：探讨PDCA（plan-do-check-act）护理模式对牙列缺损种植修复患者种植效果及心理状态的影响。方法：选取 2020 年4月-2021年4月在笔者医院进行种植修复的84例牙列缺损患者为研究对象，采用随机数字表法分为观察组（n=42） 和对照组（n=42）。对照组予以常规护理干预，观察组予以PDCA护理模式干预。比较两组护理干预后种植效果、干预前与 干预1周后状态特质焦虑问卷中状态焦虑分量表（State anxiety inventory，S-AI）、特质焦虑分量表（Trait anxiety inventory，T-AI）与汉密尔顿抑郁量表（Hamilton depression scale-17，HAMD-17）评分；干预3个月后，选择口腔健 康影响程度量表（Oralhealth impact profile-14 ，OHIP-14）评估患者口腔健康相关生活质量（Oral health related quality of life，OHRQoL）；统计两组并发症发生率。结果：观察组舒适度、咀嚼、语言、美观及固位种植效果均高于对 照组（P＜0.05）；干预1周后，观察组S-AI、T-AI与HAMD-17评分均低于对照组（P＜0.05）；干预3个月后，观察组OHRQoL 中生理障碍、残障、功能限制、生理性疼痛、心理不适、社交障碍及心理障碍评分均低于对照组（P＜0.05）；观察组患者 并发症总发生率低于对照组（P＜0.05）。结论：对牙列缺损种植修复患者予以PDCA护理模式干预可提高患者种植效果，减 少患者焦虑及抑郁心理状态，改善OHRQoL，降低术后并发症发生率。
Abstract:
Objective To explore the eff ect of PDCA nursing intervention on implant eff ect and psychological state of patients with dentition defent implant restoration. Methods 84 patients with dentition defects who underwent implant restoration in the author’s hospital from April 2020 to April 2021 were selected as the research objects and were divided into observation group (n=42) and control group (n=42) by random number table method. The control group was given routine nursing,and the observation group was given PDCA nursing mode intervention; The changes in the scores of the State Trait Anxiety Questionnaire including the state anxiety subscale (S-AI) and trait anxiety subscale (T-AI),and Hamilton Depression Scale-17 (HAMD-17) were compared between the two groups before the intervention and one week after the intervention. Oral health related quality of life ( OHRQoL) in the two groups after three months of intervention was assessed by OralHealth Impact Profi le-14 (OHIP-14 ),and the incidence of complications in the two groups was calculated. Results The comfort,mastication , language,aesthetics and retention implant eff ects of the observation group were signifi cantly higher than those of the control group (P ＜0.05). After one week of intervention,the S-AI,T-AI and HAMD-17 scores of the observation group were lower than those of the control group (P ＜0.05). After three months of intervention,the scores of physiological disorder,disability,functional limitation,physiological pain,psychological discomfort,social disorder and psychological disorder in OHRQoL of the observation group were signifi cantly lower than those of the control group (P ＜0.05). The total incidence of complications in the observation group was signifi cantly lower than that in the control group (P ＜0.05). Conclusion The PDCA nursing model intervention for patients with dentition defect implant restoration can improve the implant effect of patients, reduce the psychological state of patients with anxiety and depression, improve OHRQoL and reduce the incidence of postoperative complications.
