摘要:
目的：观察牙冠延长术联合玻璃纤维桩修复术治疗前牙残根残冠的效果。方法：选择南通市海门区人民医院口腔科 2016 年2月-2021年2月收治的80例前牙残根残冠患者，按照随机数字表法分为玻璃纤维桩组（n=40，患牙51颗，玻璃纤维桩 修复术）和联合组（n=40，患牙56颗，牙冠延长术联合玻璃纤维桩修复术），比较两组患者修复效果、美学评价、牙周指标、 相关预后指标及不良事件发生情况。结果：联合组修复成功率为98.21%，显著高于玻璃纤维桩组的88.23%（P＜0.05）。修复 前，两组患者红色美学指数（Pink esthetic score，PES）评分、白色美学指数（White esthetic score，WES）评分、探 诊深度（Probing depth，PD）、附着水平（Attachment level，AL）、龈沟出血指数（Sulcus bleeding index，SBI）、 咀嚼效率、生活质量综合评分量表-74（QOL-74）评分、疼痛数字评定量表（Numerical rating scale，NRS）评分差异均无 统计学意义（P＞0.05），修复后两组患者PES评分、WES评分、咀嚼效率、QOL-74评分均显著上升，联合组高于玻璃纤维桩 组（P＜0.05），PD、AL、SBI、NRS评分均显著下降，联合组低于玻璃纤维桩组（P＜0.05）。联合组不良事件总发生率低于 玻璃纤维桩组（P＜0.05）。结论：牙冠延长术联合玻璃纤维桩修复术治疗前牙冠残根残冠患者修复效果突出，既能提高牙 齿美观性，还能维护牙周健康、改善预后，安全性较高。 [
Abstract:
Objective The aim of this study was to observe the therapeutic effect of crown lengthening surgery combined with glass fi ber post on residual roots and crowns of anterior teeth. Methods A total of 80 patients with residual roots and crowns of anterior teeth who were admitted to the Department of Stomatology, Haimen District Peoples Hospital, Nantong from February 2016 to February 2021 were selected, and divided into glass fiber post group (n=40, 51 affected teeth) and combination group (n=40, 56 aff ected teeth) by random number table method, receiving repair with with glass fi ber post and crown lengthening surgery combined with glass fi ber post, respectively. The two groups were compared in terms of restoration results, aesthetic effect, periodontal indexes, related prognostic indicators and adverse events. Results The success rate of repair in the combination group (98.21%) was signifi cantly higher than that in the glass fi ber post group (88.23%) (P ＜0.05). The pink esthetic score (PES), white esthetic score (WES), probing depth (PD), attachment level (AL), sulcus bleeding index (SBI), masticatory effi ciency, the Generic Quality of Life Inventory-74 (QOL-74) score and Numerical Rating Scale (NRS) score showed no signifi cant diff erence between the groups before repair (P ＞0.05). After repair, the PES scores, WES scores, masticatory effi ciency and QOL-74 scores were signifi cantly increased, and higher in the combination group than in the glass fi ber post group (P ＜0.05). PD, AL, SBI and NRS scores were signifi cantly decreased, and lower in the combination group than in the glass fi ber post group (P ＜0.05). The total incidence of adverse events in the combination group was lower than that in the glass fi ber post group (P ＜0.05). Conclusion Crown lengthening surgery combined with glass fi ber post is eff ective in repairing residual roots and crowns of anterior teeth. The combined use can not only improve the aesthetic eff ect but also maintain periodontal health and improve prognosis, with high safety.
