摘要:
目的：探讨上前牙唇侧骨壁部分缺损即刻种植的疗效及对美学效果的影响。方法：选取2019年6月-2020年6月在笔者 医院口腔科行上前牙即刻种植修复的患者62例。根据唇侧骨壁完整性将患者分为完整组（唇侧骨壁完整）和缺损组（唇侧 骨壁有冠方缺损但不超过1/2），各31例，对两组均实施即刻种植。术后12个月，评价两组的种植成功率、并发症情况， 术后即刻、术后6个月，评价两组的唇侧骨壁厚度、红色美学指数（Pink esthetic score，PES）、白色美学指数（White esthetic score，WES）。结果：两组种植成功率及并发症总发生率比较，差异无统计学意义（P＞0.05）。术后6个月，两 组冠方、中部、根尖的唇侧骨壁厚度均有所减小（P＜0.05），但两组组间变化值比较，差异均无统计学意义（P＞0.05）。 术后6个月，两组PES、WES各项评分及总分均高于术后即刻（P＜0.05），但两组PES、WES评分比较，差异均无统计学意义 （P＞0.05）。结论：与唇侧骨壁完整患者即刻种植相比，唇侧骨壁部分缺损患者采取即刻种植同其疗效相当，且成功率 高、并发症少，美观度较好。
Abstract:
Objective To explore the eff ect of immediate implantation of labial bone wall partial defect of the upper anterior teeth and its infl uence on the aesthetic. Methods From June 2019 to June 2020, 62 patients who underwent immediate implant restoration of upper anterior teeth in the department of stomatology of the authors hospital were selected. According to the integrity of the labial bone wall, patients were divided into the complete group (complete labial bone wall, n=31) and the defect group (coronal defect of the labial bone wall but not more than 1/2, n=31), immediate implantation was performed in both groups. At 12 months after the operation, the implantation success rate and complications of the two groups were evaluated, at immediately after the operation and 6 months after the operation, the labial bone wall thickness, pink esthetic score (PES), and white esthetic score (WES) of the two groups were evaluated. Results There was no signifi cant diff erence in the implantation success rate and the total incidence of complications in the two groups (P＞0.05). At 6 months after the operation, the thickness of the labial bone wall at the crown, middle and root tip of the two groups decreased（P＜0.05）, but there was no signifi cant diff erence in the two groups (P ＞0.05). At 6 months after the operation, the PES and WES scores and total scores of the two groups were higher than immediately after the operation（P＜0.05）, but there was no significant difference in the PES and WES scores in the two groups (P＞0.05). Conclusion Compared with patients with intact labial bone wall, immediate implantation in patients with partial labial bone wall defect has the same effi cacy, higher success rate, fewer complications and better aesthetics.
