摘要:
目的：研究基于聆听-确立-运用-提升-反馈-强化（LERANS）模式的护理干预在先天性内眦赘皮患儿矫正治疗中的应 用效果。方法：选择2020年10月-2021年10月笔者科室收治的174例先天性内眦赘皮并行矫正术的患儿，采用随机抽签法分为 常规干预组（87例）和反馈强化组（87例）。常规干预组进行常规健康教育干预；反馈强化组在常规干预组的基础上采用基 于LERANS模式的护理干预。干预后，比较两组患者眼部疼痛、肿胀程度评分、内眦间距（Inner canthic diameter，ICD） 缩短率、睑裂长度（Length of palpebral fissure，HLFL）/ICD达标率、瘢痕评分、家长对患儿眼部美观的满意度及护理 质量评分。结果：术后3 d，反馈强化组疼痛、肿胀程度评分均低于常规干预组（P＜0.05），两组内眦间距缩短率及（睑裂 长度/内眦间距）达标率比较差异无统计学意义（P＞0.05）；干预后，反馈强化组VSS评分低于常规干预组（P＜0.05），家 长对患儿眼部美观的总满意率及护理质量评分高于常规干预组（P＜0.05），总并发症发生率低于常规干预组（P＜0.05）。 结论：采用基于LERANS模式的护理干预可有效缓解先天性内眦赘皮患儿术后疼痛、肿胀程度，减少并发症，减少瘢痕形成， 提高患儿家长对患儿眼部美观的满意度，具有较好的干预效果。
Abstract:
Objective To study the application effect of nursing intervention based on LERANS model in the correction treatment of children with congenital epicanthus. Methods A total of 174 children with congenital epicanthoplasty treated in the author’s department from October 2020 to October 2021 were selected and divided into routine intervention group (87 cases) and feedback reinforcement group (87 cases) by random drawing method. Routine intervention group performed routine health education intervention; The nursing intervention based on LERANS model was adopted in the feedback intensive group on the basis of the conventional intervention group. After the intervention,the score of eye pain,swelling degree,inner canthic diameter (ICD) shortening rate,length of palpebral fi ssure (HLFL)/ICD compliance rate,scar score,parents’ satisfaction with eye appearance and nursing quality of the children were compared between the two groups. Results On the third day after surgery,the pain and swelling degree scores of the feedback enhancement group were lower than those of the conventional intervention group (P＜0.05),and there was no statistical signifi cance in ICD shortening rate and HLFL/ICD compliance rate between the two groups (P＞0.05). After the intervention,the VSS score of the feedback intensive group was lower than that of the conventional intervention group (P＜0.05),the total satisfaction rate and nursing quality score of parents on the children’s eye appearance were higher than those of the conventional intervention group (P＜0.05),and the total complication rate was lower than that of the conventional intervention group (P＜0.05). Conclusion Nursing intervention based on LERANS model can eff ectively relieve postoperative pain and swelling of children with congenital epicanthus,reduce complications,reduce scar formation,and improve parents’ satisfaction with eye appearance of children with good intervention eff ect.
