[摘要]目的：探究皮肤镜在面部光线性角化病（AK）中的临床应用。方法：选取2017年3月-2017年12月笔者医院皮肤科收
治的疑似面部光线性角化病患者61例，所有患者均进行皮肤镜检查，以病理检测为金标准，结果显示面部光线性角化病38
例为观察组，非面部光线性角化病23例为对照组。评估皮肤镜在面部光线性角化病中的应用价值。结果：观察两组患者皮
肤镜下表现，基底假网状红斑、毛囊口周围白晕、毛囊口黄色角栓、表面黄白色鳞状块、玫瑰花瓣征、毛囊口周围粗大不
规则线状血管、毛囊口周围细小、线状-波浪状血管、周边色素加深，观察组出现的概率均高于对照组，差异有统计学意义
（P ＜0.05）。以上8个临床症状中单独诊断价值以表面黄白色鳞状块的灵敏度最高（97.37%），周边色素加深的特异度最高
（91.30%），毛囊口周围白晕的约登指数最高（0.62）。基底假网状红斑+毛囊口黄色角栓+毛囊口周围白晕三者联合诊断约
登指数最高（0.77），诊断符合率为90.16%。结论：皮肤镜对于面部光线性角化病诊断有较高的特异性表现，以毛囊口黄色
角栓、毛囊口周围白晕、基底假网状红斑这三个特征联合诊断，诊断价值最高。

Abstract:

Abstract: Objective To investigate application effect of dermoscopy in the diagnosis of facial actinic keratosis(AK). Methods
A total of 61 patients with suspected AK admitted to dermatological department of our hospital from March 2017 to December
2017 were selected. Patients were divided into two groups according to the dermoscopy results, namely observation group
(AK, n =38) and control group (non-AK, n =23). Then the application effect of the dermoscopy in facial AK was evaluated.
Results The dermoscopic features including background erythema and red pseudonetwork, hair follicle openings surrounded
by a white halo, hair follicle openings filled with yellowish keratotic plugs, white to yellow surface scales or keratin mass,
rosette sign, large irregular linear vessels surrounding the hair follicles, fine and linear-wavy vessels surrounding the hair
follicles, and peripheral pigmentation had higher appearance rate in the observation group than in the control group (P ＜0.05).
Among single detection value of the above 8 features, white to yellow surface scales or keratin mass had the highest diagnosis
sensitivity(97.37%), peripheral pigmentation had highest diagnosis specificity (91.30%), hair follicle openings surrounded
by a white halo had the highest Youden’s index (0.62).The Youden’s index of combined detection of the 3 features including
background erythema and red pseudonetwork, hair follicle openings filled with yellowish keratotic plugs, and hair follicle
openings surrounded by a white halo was highest (0.77) with an coincidence rate of 90.16%. Conclusion The diagnostic
value of dermoscopy for facial AK is relatively high, which is highest in the combined detection of the 3 features including
background erythema and red pseudonetwork, hair follicle openings filled with yellowish keratotic plugs, and hair follicle
openings surrounded by a white halo.
