[摘要]目的：研究A型肉毒毒素联合氟尿嘧啶治疗烧伤瘢痕的美容效果及对瘢痕复发的影响。方法：采用双盲法将2016年1
月-2018年6月医院门诊收治的112例烧伤瘢痕患者分为A组（A型肉毒毒素+氟尿嘧啶，n =56）与B组（曲安奈德，n =56），比
较两组治疗效果。结果：两组治疗后，温哥华烧伤瘢痕评估量表中瘢痕色素沉着、血供、柔韧性及高度得分均较同组治疗
前显著下降，且A组治疗后温哥华烧伤瘢痕评估量表中的各项得分均显著低于B组，差异有统计学意义（P＜0.05）。治疗后，两
组瘢痕痛痒程度均较同组治疗前显著下降，且A组治疗后瘢痕痛痒程度显著低于B组，差异有统计学意义（P ＜0.05）。A组治疗
效果及治疗有效率显著优于B组，差异有统计学意义（P＜0.05）。两组不良反应发生率比较差异无统计学意义（P＞0.05）。随
访1.5年，A组复发率为7.14%（4/56），B组复发率为17.86%（10/56），但两组复发率比较差异无统计学意义（χ2=2.294，
P =0.086）。结论：A型肉毒毒素联合氟尿嘧啶与曲安奈德在治疗烧伤瘢痕中均具有良好的疗效，但与曲安奈德相比，A型肉
毒毒素联合氟尿嘧啶注射在改善瘢痕美容度、痒痛症状中的效果更好，且治疗后复发率更低，疗效更好。

Abstract:
Abstract: Objective To investigate the cosmetic effects of botulinum toxin type A combined with fluorouracil in the treatment
of burn scars and the effect on scar recurrence. Methods 112 burn scar patients admitted to the hospital from January 2016 to
June 2018 were enrolled, and divided into two groups by double-blind method, 56 in each group. Group A received botulinum
toxin type A and fluorouracil, and group B received triamcinolone acetonide. Then the therapeutic effects of the two groups
were compared. Results The scores of Vancouver scar scale (VSS) and scores of the items including scar pigmentation,
vascularity, pliability and height were decreased in both groups after treatment, and the scores of group A were significantly
lower than those of group B, the differences were statistically significant(P ＜0.05). The degree of scar itching was decreased
in both groups after treatment(P ＜0.05), and The degree of scar pain and itching in group A was significantly lower than that
in group B (P ＜0.05). The therapeutic effect and treatment efficiency of group A were significantly better than those of group
B (P ＜0.05). There was no significant difference in adverse reactions between the two groups (P ＞0.05). The recurrence rate
was 7.14% (4 cases) in group A and 17.86% (10 cases) in group B during the follow-up period of 1.5 years, the difference was
not statistically significant (χ2=2.294，P =0.086). Conclusion Compared with triamcinolone acetonide, botulinum toxin type
A combined with fluorouracil has similar clinical efficacy in the treatment of burn scars, but has better cosmetic effect and itchrelief
effect, also lower recurrence rate.
