摘要:

目的：探讨眼轮匝肌缩短联合睑袋切除术与单纯眼轮匝肌缩短术对老年下睑内翻患者疗效和预后的影响。方法：选择2019年1月-2019年10月80例(126只眼)下睑内翻的老年患者作为研究对象，采用随机数字表法平均分为观察组与对照组，对照组给予单纯眼轮匝肌缩短术治疗，观察组给予眼轮匝肌缩短联合睑袋切除术。比较两组术后4周的近期疗效，1年后评价远期疗效。于术前、术后1个月、6个月、术后1年参照眼表疾病指数量表（Ocular surface disease index，OSDI）评价眼部状况，于术后1周、6个月、1年采用温哥华瘢痕量表(Vancouver scar scale，VSS)评价术后瘢痕情况，并比较两组患者满意度。结果：观察组患者术后的近期良好率及总有效率高于对照组，且远期显效率及总有效率亦高于对照组，差异有统计学意义（P＜0.05）。观察组畏光、流泪、刺激感、异物感症状较对照组改善明显，差异有统计学意义(P＜0.05)。术后6个月、1年观察组瘢痕的色泽、血管分布、厚度、柔软度均对照组轻，差异有统计学意义(P＜0.05)。观察组非常满意及总满意率高于对照组，差异有统计学意义（P＜0.05）。结论：眼轮匝肌缩短联合睑袋切除术可以更好的改善患者眼部不适症状，具有更好近期及远期疗效，且术后瘢痕恢复更佳，美容效果更好，患者满意度更高。

Abstract:

Objective To explore the effects of orbicularis oculi muscle shortening combined with blepharoplasty and simple orbicularis oculi muscle shortening on the curative effects and prognosis of senile patients with lower eyelid entropion. Methods From January 2019 to October 2019, 80 elderly patients (126 eyes) with lower eyelid entropion were selected as the research objects. All patients were randomly divided into observation group and control group. The control group was treated with simple orbicularis oculi shortening, while the observation group was treated with orbicularis oculi shortening combined with blepharoplasty. Compare the short-term efficacy 4 weeks after operation, and the long-term efficacy 1 year later. Ocular surface disease index (OSDI) was used to evaluate eye symptoms before operation, 1 month, 6 months after operation, and 1 year after operation. Vancouver Scar Scale (VSS) was used to evaluate postoperative scar 1 week, 6 months and 1 year after operation. Then compared the satisfaction of the two groups. Results The short-term good rate and the total effective rate of patients in the observation group after surgery were higher than those in the control group, and the long-term significant rate and th e total effective rate were also higher than those in the control group, and the differences were statistically significant (P＜0.05). Compared with the control group, the symptoms of photophobia, lacrimation, irritation and foreign body sensation in the observation group were significantly improved (P＜0.05). The color, vascular distribution, thickness and softness of the scar in the observation group were milder than those in the control group six months and one year after operation (P＜0.05). The very satisfactory rate and the total satisfactory rate in the observation group were higher than those in the control group, and the differences were statistically significant (P＜0.05). Conclusion Orbicularis oculi muscle shortening combined with blepharectomy can better improve the ocular discomfort in patients, which has better short-term and long-term efficacy. Besides, the postoperative scar recoveryand the cosmetic effect are also better in the observation group than that in control group. The patient satisfaction is higher in the observation group.
